摘 要:目的探讨儿童麻疹肺炎的胸部X线表现分型,并分析各型麻疹肺炎的严重程度,提高对该病的认识和诊断水平。方法回顾性分析89例儿童麻疹肺部感染的胸部X线表现。将麻疹患者的肺部感染分为四种类型,并就各型麻疹肺炎的血清乳酸脱氢酶(LDH)指标进行统计学分析。结果麻疹肺炎分型:根据麻疹患者胸部X线表现,将麻疹肺炎分为肺气肿型10例,肺间质型23例,肺小叶型30例,肺实变型26例。采用方差分析比较麻疹肺炎各型间的指标差异,结果显示,各型麻疹肺炎的LDH之间有显著性差异(F=9.117,P<0.05)。采用LSD法进行两两比较,肺间质型(t=2.071,P=0.041)、肺小叶型(t=2.116,P=0.037)、肺实变型(t=4.714,P=0.000)均高于肺气肿型。肺间质型和肺小叶型之间差异无统计学意义(t=0.042,P=0.966)。肺间质型(t=3.387,P=0.001)和肺小叶型(t=3.663,P=0.000)均低于肺实变型。结论对儿童麻疹肺炎进行分型,结合血清LDH指标进行统计学分析,有助于加深对麻疹肺炎的认识,对于指导临床治疗及疗效评估有重要意义。Objective To explore the chest X-ray manifestation and classification of measles pneumonia in children,and to analyze the severity of each type of measles pneumonia,so as to improve and attach importance to the understanding and diagnosis of this disease.Methods The chest X-ray findings of 89 children with measles pulmonary infection were retrospectively analyzed.Pulmonary infection of measles patients was divided into four types,and the s-LDH index of mea-sles pneumonia was analyzed statistically.Results The classification of measles pneumonia.According to the main chest X-ray manifestations of measles patients,measles pneumonia was classified into the following types:emphysema type in 10 cases,interstitial type in 23 cases,lobular type in 30 cases,consolidation type in 26 cases.ANOVA was used to compare the differences among different types of indicators.The results showed that:there was significant difference in s-LDH be-tween different types of measles pneumonia(F=9.117,P<0.05).LSD test was used to compare among the groups.The pulmonary interstitial type(t=2.071,P=0.041),lobular type(t=2.116,P=0.037),consolidation type(t=4.714,P=0.000)were higher than emphysema type.There was no statistically significant difference between interstitial type and lobular type(t=0.042,P=0.966).Interstitial type(t=3.387,P=0.001)and lobular type(t=3.663,P=0.000)were all lower than those of consolidation type.Conclusion Classification of measles pneumonia in children and statistical analysis of s-LDH index are helpful to deepen the understanding of measles pneumonia.It is of great significance for guid-ing clinical treatment and evaluation for curative effect.
