摘  要:目的利用动态增强磁共振成像(DCE-MRI)MR肾图动态观察兔单侧输尿管梗阻(UUO)再通术(RUUO)后肾功能恢复情况。方法将50只兔随机分5组,其中对照组进行假手术,其余4组手术建立右侧输尿管可再通梗阻模型,分别于梗阻后第3、7、14、28天行输尿管再通术,均于再通当天术前以及术后1个月行DCEMRI绘制时间-强度曲线(即MR肾图),并根据Baumann-Rudin模型计算肾小球滤过系数(kcl)。结果与对照组比较,UUO术后3天患肾kcl明显下降(P<0.05);7天和14天组kcl较3天组缓慢下降(P>0.05);28天组较14天组kcl再次显著下降(P<0.05);除28天组外,3天、7天和14天组RUUO术后一个月与术前比较,kcl均明显恢复(P<0.05)。结论DCE-MRI能反映梗阻性肾病肾功能变化;梗阻解除时间是影响肾功能恢复的重要因素。Objective Dynamic contrast-enhanced MR nephrography was used to observe the recovery of renal function after unilateral ureteral obstruction recanalization in rabbits.Method Fifty rabbits were randomly divided into five groups.Sham operation was performed in the control group,and right ureteral recanalization obstruction models were estab-lished in the other four groups.Ureteral recanalization was performed on the 3rd,7th,14th and 28th day after obstruction.Dynamic enhanced MR was used to draw time-intensity curve(MR nephrogram)before and 1 month after recanalization.Glomerular filtration coefficient was calculated according to Baumann-Rudin model.Result Compared with the control group,the kcl of kidney in UUUO group decreased significantly 3 days after operation(P<0.05);kcl decreased slowly after 7d and 14d compared with 3d(P>0.05);kcl decreased significantly again after 28 days compared with 14 days(P<0.05);except for 28 days group,kcl of RUUO in 3 days,7 days and 14 days group recovered significantly one month after operation(P<0.05).ConclusionDynamic contrast-enhanced MR can reflect the changes of renal function in obstructive nephropathy,and the relief time of obstruction is an important factor affecting the recovery of renal function.
