青年勃起功能障碍患者性激素异常分布研究

摘　　要：目的观察20~40岁青年勃起功能障碍(erectile dysfunction,ED)患者性激素水平及异常性激素分布情况,包括:总睾酮(total testosterone,TT)、泌乳素(prolactin,PRL)、卵泡刺激素(follicle stimulating hormone,FSH)、黄体生成素(luteinizing hormone,LH)、雌二醇(estradiol,E2)。方法纳入644例20~40岁ED患者,测定其性激素水平。以TT>8nmol/L、PRL≤735mIU/L、FSH[1.27~19.26]mIU/mL、LH[1.24~8.62]mIU/mL、E2[<73,252.49]pmol/L为正常范围。观察ED患者中性激素水平异常比例。结果年龄:29.97(27.00~33.54)岁。TT:14.06(11.32~17.07)nmol/L,≤8nmol/L占5.43%(35例)。FSH:4.05(2.93~5.89)m IU/m L,异常占2.80%(18例)。LH:3.56(2.65~4.79)m IU/m L,异常占4.35%(28例)。PRL:187.90(140.24~253.49)m IU/L,重度高泌乳素血症(severe hyperprolactinemia,SHPRL)PRL>735mIU/L占0.93%(6例)。E2:136.87(103.31~177.42)pmol/L,>252.49pmol/L占3.88%(25例)。原发性性腺功能减退0.15%(1例),继发性性腺功能减退0.78%(5例)。TT、E2、SHPRL合计异常比例为9.47%(61例)(含多项异常值)。结论性激素水平异常在青年ED患者中比例较低.Objective To investigate the abnormal distribution of sex hormone levels in young man(20~40 years old)with erectile dysfunction(ED), including total testosterone(TT), prolactin(PRL), follicle stimulating hormone(FSH),luteinizing hormone(LH), estradiol(E2). Methods A total of 644 cases with ED aged between 20 to 40 years old were enrolled in the study. The levels of sex hormone were measured. TT>8 nmol/L, PRL≤735 mIU/L, FSH[1.27~19.26]mIU/mL,LH[1.24~8.62]mIU/mL, E2[<73,252.49]pmol/L were defined as the normal range. The abnormal distribution of sex hormone levels was observed in males with ED. Results Average age of 644 young males with ED was 29.97(27.00~33.54)years old.Low TT, severe hyperprolactinemia(SHPRL)(PRL>735 mIU/L),abnormal FSH and LH, elevated E2 accounted for 5.43%(35 cases), 0.93%(6 cases), 2.80%(18 cases),4.35%(28 cases) and 3.88%(25 cases), respectively. Primary hypogonadism and Secondary hypogonadism accounted for 0.15%(1 case) and 0.78%(5 cases). Total cases of low TT, SHPRL and elevated E2 accounted for 9.47%(61 cases)(including patients with multiple abnormal values). Conclusion Abnormal sex hormone levels account for low percentage in young man with ED.
