摘  要:目的探讨在1.5 T磁共振3D高分辨率对比增强神经成像(3D CE-MRN)对正常三叉神经颅外段分支的显示效果。方法经华中科技大学同济医学院医学伦理委员会批准,征集2017年10月至2018年2月35名成年健康志愿者,男22名,女13名,年龄26~68岁,平均47.4岁,在Siemens Aera 1.5 T超导型磁共振上行三维短时反转恢复快速自旋回波成像(3D STIR SPACE)CE-MRN,钆喷酸葡胺(Gd-DTPA)注射量为0.15mmol/kg体重。由两名放射科医师分别对原始数据进行处理,并分别评分。清晰显示神经主干的完整走行得3分;清晰显示神经主干走行的大部分得2分;对神经主干走行显示模糊或只能节段显示得1分,神经主干不可分辨得0分。使用Kappa检验评价两组评分的一致性,统计三叉神经各分支的清晰完整显示率和清晰大部分显示率。结果两组评分一致性好,Kappa值均>0.75。眼神经、上颌神经、耳颞神经、舌神经、下牙槽神经的清晰完整显示率分别为77%、70%、85%、84%、99%;清晰大部分显示率分别为89%、89%、97%、99%、100%。结论 CE-MRN序列可以清晰显示三叉神经颅外段分支。三叉神经颅外段神经成像在1.5 T机型中也可以达到很好的效果。Objective To study the visualization of the extracranial branches of normal trigeminal nerve by 3 D high resolution contrast-enhanced neurography on 1.5 T MR scanner. Methods After approval by the Medical Ethics Committee of Tongji Medical College of Huazhong University of Science and Technology,we recruited 35 healthy adult volunteers(aged 26 to 68;average age,47.4 years;22 male and 13 female) from October 2017 to February 2017. All volunteers received a contrast-enhanced 3 D STIR SPACE neurography in a Siemens Aera 1.5 T MR. The Gd-DTPA injection quantity was 0.15 mmol per kilogram. The original data were processed and graded by two radiologists. Nerves which were distinct and showed the complete structure scored 3 points;nerves which were distinct and showed most of the structure scored 2 points;nerves which were fuzzy or show little structure score 1 point,and if the nerve can not be identified it scored 0 point. Inter-observer agreement between the two radiologists in imaging evaluation was determined with kappa statistics. Results Inter-observer agreement was considered to be good(the Kappa statistics were all >0.75). The rates at which the ophthalmic nerve,the maxillary nerve,the auriculotemporal nerve,the lingual nerve and the inferior alveolar nerve got 3 points were 77%,70%,85%,84% and 99%,respectively. The rates at which of the ophthalmic nerve,the maxillary nerve,the auriculotemporal nerve,the lingual nerve and the inferior alveolar nerve get 2 points or 3 points were 89%,89%,97%,99%,100%,respectively. Conclusion The CE-MRN sequence can clearly show the extracranial branches of the trigeminal nerve. The CE-MRN sequence for the trigeminal nerve is also effective in 1.5 T MR scanner.
