摘  要:目的分析比较256 iCT不同低辐射剂量扫描条件联合高级迭代重组算法(iDose4)对头颈部CT血管造影(CTA)图像质量及辐射剂量的影响。方法前瞻性纳入2016年10月至2017年10月行头颈部CTA检查的患者150例,随机分为A^E 5组,每组30例。A组行常规剂量扫描,管电压120 kV,管电流250 mAs,采用滤波反投影法(FBP)重组;B、C、D、E组行低剂量扫描,B组行管电压120 kV,低管电流150 mAs扫描,C组行管电压120 kV,低管电流100 mAs扫描,D组行低管电压100 kV,管电流250 mAs扫描,E组行低管电压100 kV,低管电流150 mAs扫描,B组、C组、D组及E组均采用iDose4,迭代重组级别5。测量各组患者颈部CTA检查颈总动脉(CCA)、颈内动脉C1段(ICA C1)、椎动脉V1段(VA V1)3个水平的动脉CT值、肌肉CT值、SD噪声值,计算信噪比(SNR)、对比噪声比(CNR),对图像质量进行主观评价,并计算辐射剂量。采用单因素方差分析比较各组图像的客观评价指标,采用Kruskal-Wallis检验比较各组图像的主观评分及辐射剂量。结果 CCA、VA V1水平SD噪声 E组与B组、C组、D组比较差异均有统计学意义(P<0.05),CCA水平SD噪声 E组与A组比较差异有统计学意义(P<0.05)。CCA、VA V1、ICA C1水平的SNR、CNR D组分别与A组、B组、C组及E组比较差异均有统计学意义(P<0.05)。头颅、颈部、肩关节3个水平的主观评分5组组间差异均无统计学意义(P>0.05),3个水平的主观评分均为D组最高。B组、C组、D组、E组辐射剂量与A组比较均有不同程度地降低,且5组的辐射剂量两两比较差异均有统计学意义(P<0.05),E组辐射剂量最低。结论 256 iCT不同低辐射剂量扫描条件能不同程度地降低头颈部CTA辐射剂量,联合iDose4均能获得满足临床诊断需求的图像。E组采用低管电压100 kV,低管电流150 mAs扫描,辐射剂量最低,联合iDose4,图像质量与常规剂量图像相当,能够满足临床诊断;D组采用低管电压100 kV,管电流250 mAs扫描,辐Objective To analyze and compare the effects of 256 iCT different low-dose scanning combined with iDose^4 on image quality and radiation dose at head and neck CTA. Methods 150 patients who underwent head and neck CTA from October 2016 to October 2017,were randomly divided into group A(conventional group,120 kV,250 mAs,filter back projection reconstruction) and groups B,C,D,E(low-dose group,iDose^4,5 levels of iterative reconstruction),Group B:120 kV,150 mAs,Group C:120 kV,100 mAs,Group D:100 kV,250 mAs,Group E:100 kV,150 mAs. We measured craniocervical different levels of CT attenuation and SDnoise,calculated the signal to noise ratio(SNR),contrast to noise ratio(CNR),evaluated the subjective image quality,and calculated the radiation dose. One-way ANOVA was used to compare the objective image quality between groups,subjective image quality and radiation dose were assessed with Kruskal-Wallis test. Results There were significant differences between group E and groups B,C,D in SDnoise value of CCA and VA V1(P<0.05);and there were significant differences between group E and group A in SDnoise value of CCA(P<0.05). There were significant differences between group D and groups A,B,C,E in SNR and CNR of CCA,ICA C1,VA V1(P<0.05). There were no significant differences in the subjective scores among groups A,B,C,D and E(all P>0.05);the subjective scores of 3 levels in group D were highest. Compared with group A,the radiation of groups B,C,D,E were reduced to a certain extent,there were significant differences between groups A,B,C,D,E(P<0.05),the radiation dose of group E was the lowest. Conclusion Under the circumstances of 256 iCT different low-dose scanning at head and neck CTA,the radiation dose decreased in different degrees,and the technique of low-dose scanning with iDose^4 could obtain satisfactory image quality. The radiation dose of low tube voltage 100 kV and low tube current 150 mAs scanning of the study was the lowest,and combining with iDose^4 iterative reconstruction,image quality could satisfy the clinical
