摘  要:目的研究全血在采集后快速降温至4-6℃,再储存,不同时间段制备的新鲜冰冻血浆(FFP)中凝血因子Ⅷ(FⅧ)和纤维蛋白原(Fib)含量的变化。方法随机选取无偿献血全血200 mL(7 min内采完)30份和400 mL(13 min内采完)150份,分为实验组和对照组(每组90份)。实验组全血采集后,降温至4-6℃再置于2-6℃储血冰箱;对照组全血采集后直接放入2-6℃储血冰箱。首先,观察2组全血降温至4-6℃所需要的时间;然后,2组均在采血后<6、6-12、12-18、18-24 h制备FFP,制备后2周内检测FⅧ含量和Fib含量。结果实验组与对照组的全血分别在采血后约3、16-18 h温度降至4-6℃;实验组与对照组FⅧ含量均随着制备时间延长而递减,差异有统计学意义(P<0.01);而2组Fib含量在24 h内均无统计学差异(P>0.05)。实验组在18-24 h时段FⅧ含量明显高于对照组(P<0.05)。另外,实验组合格率在80%以上,对照组12-18、18-24 h时段的合格率仅73%和53%,显著低于实验组(P<0.01)。结论全血快速降温至4-6℃,并维持2-6℃环境下储存,可延缓FⅧ下降,延长制备FFP时间至18-24 h,提高FFP的产量。Objective In this study,we rapidly cooled down the whole blood to 4-6℃ prior to freezer storage and following fresh frozen plasma(FFP) preparation, so as to investigate how pre-storage cooling and different times of preparation effect Factor Ⅷ(FⅧ) and fibrinogen(Fib) contents in FFP. Methods Thirty units of 200 mL whole blood(collected within 7 min) and 150 units of 400 mL whole blood(collected within 13 min) from voluntary blood donors were randomly chosen and divided into experimental group and control group(90 units/group).The units in control group were put into 2-6℃ freezer directly after collection, while units in experimental group were cooled rapidly to 4-6℃within 3 h after collection prior to 2-6℃ freezer storage. The time that whole blood required for cooling down to 4-6℃ in both groups was recorded. FFP prepared from the whole blood units in both groups at various times during storage(including <6, 6~12, 12~18 and 18~24 h after collection) was sampled for FⅧand Fib measurements. Results The time that whole blood required for cooling down to 4-6℃ in experimental group and control group was 3 and 16-18 hours, respectively. As the storage time prolonged, the decline in FⅧ contents in FFP prepared in experimental group was significantly delayed than that in the control group(P<0.01);but the results also showed no significant difference in Fib contents between two groups during the 24-hour storage(P>0.05). The content of FⅧ prepared at 18-24 h after collection was significantly higher in experimental group compared with the control group.The overall passing rate of FFP in experimental group was above 80%, while that of FFP prepared at 12~18 h and 18~24 h in control group was only 73% and 53%, respectively, which was significantly lower than that of the experimental group(P<0.01).
