摘 要:目的了解2018年D8基因型麻疹病毒(Measles virus,MV)病例的流行病学和病原学特征。方法通过中国麻疹监测信息报告管理系统收集上海市2018年麻疹病例;对病原学标本进行MV核酸检测、病毒分离和基因测序定型;对数据进行描述性分析。结果2018年上海市鉴定发现7例D8基因型MV病例,其中3月发病2例、10-12月5例;6月龄1例、25-31岁6例;无含麻疹成分疫苗(MCV)免疫史2例、免疫史不详5例。7例病例属于3起聚集性疫情:在出疹前7-21d有泰国旅行史2例;有日本旅行史2例及其密切接触感染1例;无外出旅行史1例及其密切接触感染1例。该7株D8基因型MV与本市2016年输入性D8基因型MV的核苷酸、氨基酸序列同源性分别为99.1%-99.3%、98.0%-98.6%;与2015-2018年其他国家D8基因型MV的同源性为99.34%-100%。结论2018年上海市发现的D8基因型MV病例为输入病例或输入相关病例。建议继续做好MCV接种工作,并开展高质量的流行病学和病原学监测。Objective To determine epidemiological and etiological characteristics of genotype D8 measles virus(MV)cases in 2018.Methods We used the Chinese Measles Surveillance Inf ormation Reporting Management System to obtain data on measles cases in 2018 in Shanghai.We tested pathogenic samples for MV nucleic acid,virus isolation,and gene sequencing,and conducted a descriptive data analysis.Results Seven genotype D8 MV cases were identified in 2018 in Shanghai:2 cases occurred in March and 5 occurred during October to December;1 case was 6 months old and 6 were 25-31 years old;2 cases had not received measles containing vaccine(MCV)and 5 cases’vaccination status was unknown.The 7 cases were f rom 3 clusters:2 cases had traveled to Thailand 7-21 days before rash onset;2 cases had traveled to Japan with 1 case infected by a close contact;1 case had no travel history and 1 case had been infected by a close contact.These 7 strains of D8 genotype MV had nucleotide homologies of 99.1%-99.3%and amino acid homologies of 98.0%-98.6%compared with imported D8 genotype MVs in Shanghai in 2016,with a 99.34%-100%homology compared with D8 genotype MVs in other countries during 2015-2018.Conclusion D8 genotype MV cases in 2018 in Shanghai were imported cases or importation-related cases.We suggest maintaining high MCV coverage and conduct high-quality epidemiological and etiological surveillance.
