摘 要:目的探讨克拉霉素辅佐莫西沙星治疗耐多药肺结核患者的临床疗效。方法选取2016年4月至2018年5月于沈阳市胸科医院治疗的耐多药肺结核患者87例作为研究对象,按治疗方法不同分为对照组(43例)与试验组(44例)。对照组单纯使用莫西沙星治疗,试验组在对照组治疗基础上采用克拉霉素治疗,比较两组临床疗效、痰菌转阴率、病灶吸收有效率、空洞缩小率、免疫细胞水平。结果试验组患者治疗有效率显著高于对照组,差异有统计学意义(P<0.05);试验组患者治疗3及6个月后的痰菌转阴率、病灶吸收有效率、空洞缩小率均高于对照组,差异有统计学意义(P<0.05);试验组患者治疗6个月后CD3^+、CD4^+、CD8^+、CD4^+/CD8^+均显著高于对照组,差异有统计学意义(P<0.05)。结论采用克拉霉素辅佐莫西沙星治疗耐多药肺结核效果确切,在痰菌转阴率、病灶吸收有效率、空洞缩小率、提高免疫功能方面具有明显优势。Objective To explore the clinical effects of clarithromycin and moxifloxacin in the treatment of multidrug-resistant tuberculosis(MDR-TB) patients. Methods A total of 87 MDR-TB patients who were treated in Shenyang Chest Hospital from April 2016 to May 2018 were selected as the study objects. According to different treatment methods, they were divided into control group(43 cases) and experimental group(44 cases). The control group was treated with moxifloxacin alone, and the experimental group was treated with clarithromycin on the basis of the control group. The clinical effects, negative conversion rate sputum bacteria, effective absorption rate of focus, cavity shrinking rate and immune cell level of the 2 groups were compared. Results The effective rate of treatment in the experimental group was significantly higher than that in the control group(P<0.05);the negative conversion rate of sputum bacteria, the effective rate of focus absorption and the rate of cavity reduction in the experimental group were significantly higher than those in the control group(P<0.05);the CD3^+, CD4^+, CD8^+, CD4^+/CD8^+ in the experimental group were significantly higher than those in the control group(P<0.05). Conclusion Clarithromycin assisting moxifloxacin is effective in the treatment of MDR-TB. It has obvious advantages in negative conversion rate of sputum bacteria, effective absorption rate of focus, cavity reduction rate and improvement of immune function.
