彩色多普勒超声监测胎儿脑动脉血流动力学在诊断胎儿生长受限中的应用价值

摘　　要：目的:讨论彩色多普勒超声监测胎儿脑动脉血流动力学在诊断胎儿生长受限中的应用价值。方法:回顾性分析经彩色多普勒超声检查后确定为怀有健康胎儿的孕34~36周的孕产妇100例资料作为健康组,另选取确定为胎儿生长受限的孕34~36周的孕产妇100例资料作为FGR组。借助彩色多普勒超声对胎儿脑动脉血流动力学进行监测,观察两组胎儿脑动脉血流动力学指标及FGR组不同妊娠结局下胎儿脑动脉血流动力学指标情况。结果:FGR组胎儿脑动脉血流动力学指标均明显低于健康组(P<0.05);FGR组中异常妊娠胎儿脑动脉血流动力学指标均明显低于正常妊娠(P<0.05)。结论:采用彩色多普勒超声能够发现孕晚期(孕34~36周)生长受限胎儿的脑动脉血流动力学指标降低现象,且生长受限胎儿中异常妊娠胎儿的脑动脉血流动力学指标更低。Objective: To discuss the application value of color Doppler ultrasound in the diagnosis of fetal growth restriction by monitoring cerebral artery hemodynamics. Method: Retrospective analysis was performed on 100 cases of pregnant women with healthy fetuses at 34 to 36 weeks of gestation determined by color Doppler ultrasound as the healthy group, and another 100 cases of pregnant women with fetal growth restriction at 34 to 36 weeks of gestation were selected as the FGR group. All pregnant women used color Doppler ultrasound to monitor the level of fetal cerebral arterial hemodynamics. The indexes of fetal cerebral artery hemodynamics in the two groups and in the FGR group with different pregnancy outcomes were observed. Result: The indexes of fetal cerebral artery hemodynamics in the FGR group were significantly lower than those of the healthy group(P<0.05). And in the FGR group, the indexes of fetal cerebral artery hemodynamics with abnormal pregnancy were significantly lower than those of the normal pregnancy(P<0.05). Conclusion: Color Doppler ultrasound can be found reduced phenomenon of the cerebral artery hemodynamics of the fetus with growth restriction during the third trimester of pregnancy(34-36 weeks of gestation), and in fetuses with growth restriction, the indexes of fetal cerebral artery hemodynamics of the fetuses with abnormal pregnancy are lower.
