多层螺旋CT与彩色多普勒超声在老年血管源性急腹症诊断中的应用价值

摘　　要：目的:探讨多层螺旋CT与彩色多普勒超声检查在老年血管源性急腹症临床诊断中的应用价值。方法:选取2017年1月-2019年1月笔者所在医院急诊收入的疑似血管源性急腹症的125例老年患者作为研究对象。所有患者入院后均接受腹部多层螺旋CT检查与腹部彩色多普勒超声检查,获取诊断结果后与临床最终诊断结果(以手术或治疗结局为准)比较,对比多层螺旋CT与彩色多普勒超声在老年血管源性急腹症诊断中的应用价值差异。结果:临床最终诊断结果明确血管源性急腹症患者共112例,非血管源性急腹症患者13例。多层螺旋CT诊断老年血管源性急腹症阳性预测值与彩色多普勒超声检查结果比较差异无统计学意义(P>0.05),但多层螺旋CT诊断特异度、灵敏度、阴性预测值及总符合率均高于超声检查结果,差异均有统计学意义(P<0.05)。结论:多层螺旋CT与彩色多普勒超声均可用于老年血管源性急腹症的早期临床诊断中,具有无医源性损伤、检查速度快、可重复检查等优点,但多层螺旋CT对老年血管源性急腹症诊断的灵敏度与正确率更高,可为临床诊断与治疗提供更为准确的参考信息。Objective:To explore the clinical value of multi-slice spiral CT and color Doppler ultrasound in the diagnosis of senile vascular acute abdomen.Method:From January 2017 to January 2019,125 elderly patients with suspected vasculogenic acute abdomen were selected as the study subjects.All patients received abdominal multi-slice spiral CT examination and abdominal color Doppler ultrasonography after admission.The diagnostic results were compared with the final clinical diagnosis(surgery or treatment outcome),the application value of multi-slice spiral CT and color Doppler ultrasonography in the diagnosis of vascular acute abdomen in the elderly were compared.Result:Clinical final diagnosis proved that there were 112 patients with hematogenous acute abdomen and 13 patients with non-vascular acute abdomen.There was no significant difference in predictive value between multi-slice spiral CT and color Doppler ultrasound in the diagnosis of senile vasogenic acute abdomen(P>0.05).But the diagnostic specificity,sensitivity,negative predictive value and total coincidence rate of multi-slice spiral CT were higher than those of color Doppler ultrasound,the differences were statistically significant(P<0.05).Conclusion:Both multi-slice spiral CT and color Doppler ultrasound can be used in the early clinical diagnosis of senile vascular acute abdomen.They have the advantages of no iatrogenic injury,rapid examination and repeatable examination.However,multi-slice spiral CT has higher sensitivity and accuracy in the diagnosis of senile vascular acute abdomen,and can provide more accurate reference information for clinical diagnosis and treatment.
