知信行教育干预模式对提高老年高血压并动脉粥样硬化患者自我管理能力的作用

摘　　要：目的:探讨知信行健康教育模式对老年高血压并动脉粥样硬化(AS)患者自我管理能力的影响。方法:选取2016年1月-2018年2月笔者所在中心接受上级医院下转老年高血压并AS住院患者83例,随机数字表法分组。对照组41例给予常规慢病患者健康管理,观察组42例给予常规慢病患者健康管理并联合知信行教育干预模式。随访3个月,比较两组自我管理能力、自我效能感、血压控制情况。结果:干预后,观察组自我管理能力、自我效能感总分及各维度评分均高于对照组,观察组血压水平低于对照组、血压控制达标率高于对照组,差异均有统计学意义(P<0.05)。结论:知信行教育模式可显著提高老年高血压并AS患者的自我管理能力,有效改善血压控制情况。Objective: To explore the effect of knowledge, belief and practice health education model on self-management ability of elderly patients with hypertension and atherosclerosis(AS). Method: From January 2016 to February 2018, 83 hospitalized elderly patients with hypertension and AS were randomly divided into two groups. In the control group, 41 patients with chronic diseases were given health management. In the observation group, 42 patients with chronic diseases were given health management and knowledge, belief and practice health education model. The patients were followed up for 3 months. The selfmanagement ability, self-efficacy and blood pressure control were compared between the two groups. Result: After intervention, the total scores of self-management ability, self-efficacy and various dimensions of the observation group were higher than those of the control group, the blood pressure level of the observation group was lower than that of the control group, and the rate of blood pressure control reaching the standard was higher than that of the control group, the differences were statistically significant(P<0.05). Conclusion: The knowledge, belief and practice education model can significantly improve the self-management ability of elderly patients with hypertension and AS, and effectively improve blood pressure control.
