摘 要:目的探讨应用心肺运动试验(CPET)制定的运动处方指导下的有氧联合抗阻运动对冠状动脉粥样硬化性心脏病(冠心病)经皮冠状动脉介入治疗(PCI)患者运动耐量和心脏功能的影响。方法纳入2018年1月至2018年12月于华中阜外医院住院的成功接受PCI的冠心病患者98例,根据患者意愿分为观察组(49例)和对照组(49例)。对照组给予标准化的药物治疗及健康教育,观察组在其基础上予以CPET制定的运动处方指导下的运动训练,共干预6个月。两组患者干预前后均进行心肺运动试验评估运动耐量及心脏功能。结果①与对照组相比,观察组患者干预后峰值摄氧量、无氧阈、峰值氧脉搏、峰值代谢当量、ΔVO2/ΔWR有明显改善,差异有统计学意义。②观察组患者干预后心功能分级优于对照组,差异有统计学意义。结论应用CPET评估心肺功能,制定个体化运动处方,指导冠心病PCI后患者进行康复运动训练,能有效改善患者的运动耐量及心脏功能。Objective To investigate the effects of aerobic combined resistance exercise under the guidance of cardiopulmonary exercise testing(CPET)on exercise tolerance and cardiac function in patients with coronary heart disease percutaneous coronary intervention(PCI).Methods Ninety-eight patients with coronary heart disease who were admitted to the hospital between January 2018 and December 2018 and accepted PCI were divided into observation group(49 cases)and control group(49 cases)according to their wishes.The control group was given standardized drug treatment and health education,and the observation group was given exercise training under the guidance of the exercise prescription formulated by CPET on the basis of the intervention for a total of 6 months.Cardiopulmonary exercise tests were performed before and after intervention in both groups to assess exercise tolerance and cardiac function.Results①Compared with the control group,the peak oxygen uptake,anaerobic threshold,peak oxygen pulse,peak metabolic equivalent,ΔVO2/ΔWR of the observation group were significantly improved after intervention,and the difference was statistically significant.②The cardiac function of the observation group was better than that of the control group after intervention,and the difference was statistically significant.Conclusion Using CPET to evaluate cardiopulmonary function,formulate individualized exercise prescriptions,and guide patients undergoing rehabilitation training after PCI for coronary heart disease can effectively improve patients'exercise tolerance and cardiac function.
