摘 要:胃肠功能衰竭是急危重症病人常见的合并症,常表现为胃潴留和肠内营养不耐受,既往对危重病人营养负平衡的忽视和对胃肠功能评价方法的欠缺,极大程度影响病情发展和预后。急重症超声将急救医学理念和超声技术完美结合,逐渐成为监护病房床旁常规设备。近期研究发现超声监测胃残余量(GRV)可较好的评估胃潴留和胃肠动力学状况,其精准性不亚于"金标准"CT且较后者更方便应用于床旁,成为近年来一大热点。本文就床旁超声对胃肠功能的定性和定量监测方法和价值进行综述。Gastrointestinal failure, often manifested by gastric retention and intolerance of enteral nutrition, is a common complication in critically ill patients. In the past, the neglect of negative balance of nutrition and the lack of evaluation methods of gastrointestinal function greatly affected the prognosis of critically ill patients. Critical ultrasound combined the knowledge of emergency medicine and the technology of ultrasound perfectly, and gradually became the bedside routine equipment in ICU. Recently, assessment of gastrointestinal dynamics by monitoring of gastric residue volume(GRV) with ultrasound have been developed. Its accuracy is no less than the "gold standard" CT, and more convenient for bedside. This review focused on the methods and value of bedside ultrasound in qualitative and quantitative monitoring of gastrointestinal function in the ICU.
