丙种球蛋白治疗重症手足口病的临床观察

摘　　要：目的观察并分析丙种球蛋白治疗重症手足口病的临床特征。方法回顾性地收集和分析惠州市中心人民医院儿科从2016年7月至2018年12月期间收治的确诊为肠道病毒71型和柯萨奇A组16型感染的重症手足口病,收集患儿发热、皮疹、治愈时间和病情严重分级等临床特征和年龄、性别、体格生长情况及病原体等可能影响临床特征的因素。结果重症手足口病患儿的性别、年龄和致病病毒种类与发热和皮疹时间不存在统计学差异(P>0.05);重症手足口病患儿的体格生长情况与发热和皮疹时间存在统计学差异(P>0.05),体格生长情况越差,发热时间和皮疹时间就越长;重症手足口病患儿的性别和年龄与患儿治愈时间和病情严重分级不存在统计学差异(P>0.05);重症手足口病患儿的体格生长情况和致病病毒种类与患儿治愈时间和病情严重分级存在统计学差异(P>0.05);体格生长情况越差,患儿治愈时间就越长,病情严重分级越严重;EV71感染患儿治愈时间长、病情严重分级越严重。结论重症手足口病患儿的体格生长情况与治疗效果和患儿症状相关,所感染病毒种类与治疗效果相关。临床应该对重症患儿进行针对性的营养支持以及对EV71感染重症手足口病给予更为积极的治疗。Objective To observe and analyze the clinical characteristics of the effect of gamma globulin on severe hand,foot and mouth disease.Methods Children with severe hand,foot and mouth disease diagnosed as enterovirus 71 and Coxsackie A type 16 infection in Pediatrics of Huizhou Central people s Hospital from July 2016 to December 2018 were collected and analyzed retrospectively.The clinical characteristics,such as fever,rash,cure time and severity grade,as well as the factors that may affect the clinical characteristics,such as age,gender,physical growth and pathogen were collected.Results There was no statistically significant difference in fever and rash time between those with different gender,age,pathogenic virus type in children with severe HFMD(P>0.05).There was a statistical difference between physical growth and fever and rash time in children with severe HFMD(P>0.05).The worse the physical growth was,the longer the fever and rash lasted.There was no significant difference between gender and age and cure time and severity grade in children with severe HFMD(P>0.05).There were statistically significant differences in physical growth,pathogenic virus type and cure time and severity grade in children with severe HFMD(P>0.05),The worse the physical growth is,the longer the treatment lasts and the more severe the disease is,the longer the treatment lasted and the more severe the grade of EV71 infection was.Conclusion Physical growth of children with severe hand,foot and mouth disease are related to the therapeutic effect and symptoms,and the type of virus infection is related to the therapeutic effect.In clinical practice,targeted nutritional support should be provided to children with severe HFMD and more active treatment should be given to children with severe hand,foot and mouth disease infected by EV71.
