摘 要:目的探讨2型糖尿病患者高尿酸血症的患病率及血尿酸水平与肾功能的相关性。方法回顾性分析2018年1月至2018年12月在西安交通大学第一附属医院住院治疗的2型糖尿病患者的基本人口学资料和临床资料。计量资料采用均数±标准差表示,计数资料的描述采用例数和百分比。采用Pearson相关分析和Logistics回归分析研究血尿酸水平和肾功能的相关性。结果808例2型糖尿病患者的平均年龄为(54.81±14.26)岁,其中男性525例(64.98%),女性283例(35.02%),2型糖尿病合并高尿酸血症的患病率为16.46%。Pearson相关分析结果显示,2型糖尿病患者血尿酸水平与肾小球滤过率呈负相关(r=-0.181,P<0.001)。多因素Logistic回归结果显示,血尿酸为253~309、309~368、>368μmol/L各组患慢性肾脏病的风险分别是1.55(95%CI:0.71~3.37)、4.03(95%CI:1.89~8.49)和7.09(95%CI:3.37~14.89)。结论2型糖尿病患者血尿酸水平与肾小球滤过率呈负相关,血尿酸水平升高与慢性肾脏病患病率增加相关。Objective To investigate the prevalence of hyperuricemia in type 2 diabetic patients and explore the correlation between the level of serum uric acid and kidney functions in type 2 diabetic patients.Methods The demographic and clinical data of type 2 diabetic patients treated in The First Affiliated Hospital of Xi'an Jiaotong University from January 2018 to December 2018 were collected and retrospectively analyzed.Measurement data were described by mean±SD,and enumeration data were presented by percentage.Pearson correlation and Logistic regression were performed to evaluate the correlation between the level of serum uric acid and kidney functions.Results Of all the 808 type 2 diabetic patients aged 54.81±14.26 years,525(64.98%)were males and 283(35.02%)were females.The prevalence of hyperuricemia in type 2 diabetic patients was 16.46%.Pearson correlation analysis showed that the level of serum uric acid was negatively associated with estimated glomerular filtration rate(eGFR)(r=-0.181,P<0.001).Multivariate Logistic regression analysis showed that compared with the level of serum uric acid in the first quartile group(<253μmol/L),the multivariate-adjusted odds ratio of chronic kidney disease(CKD)in the second(253-309μmol/L),third(309-368μmol/L)and fourth(>368μmol/L)quartile group was 1.55(95%CI:0.71-3.37),4.03(95%CI:1.89-8.49)and 7.09(95%CI:3.37-14.89),respectively.Conclusion The level of serum uric acid is negatively associated with eGFR.High level of serum uric acid is associated with increased prevalence of CKD.
