摘 要:目的:探讨改良鼻饲法在ICU患者肠内营养中的应用效果。方法:将选取的100例ICU患者随机分为2组,观察组给予改良鼻饲法,对照组常规鼻饲,观察并比较2组患者营养状况、血糖水平、鼻饲相关并发症的差异。结果:肠内营养支持2周后,观察组与对照组血清白蛋白、血清总蛋白均显著低于治疗前(P<0.05),但观察组鼻饲2周后血清白蛋白、血清总蛋白水平均高于对照组(P<0.05);观察组与对照组鼻饲前后血红蛋白水平均没有显著差异(P>0.05);观察组与对照组鼻饲1周血糖水平、鼻饲2周血糖水平均显著低于鼻饲第1天(P<0.05),但观察组鼻饲1周血糖水平、鼻饲2周血糖水平均显著低于对照组(P<0.05);肠内营养支持2周后,观察组呕吐、高血糖发生率显著低于对照组(P<0.05),两组反流、腹泻发生率均没有显著差异(P>0.05)。结论:改良鼻饲法有利于减缓ICU患者营养状况恶化,有利于ICU患者控制血糖水平,有利于减少鼻饲相关并发症的发生。Objective To investigate the effect of modified nasal feeding on enteral nutvition in ICU patients.Method Totally 100 intensive care unit patients were randomly divided into two groups.The observation group was given improved nasal feeding,and the control group was given routine nasal feeding.The nutritional status,blood glucose level,and nasal feeding related complications were observed and compared between the two groups.Result After 2 weeks of enteral nutrition,serum albumin and serum total protein in the observation group and control group were significantly lower than before treatment(P<0.05),but serum albumin and serum total protein levels in the observation group were higher than the control group after nasal feeding for 2 weeks(P<0.05).There was no significant difference in hemoglobin levels between the observation group and the control group before and after nasal feeding(P>0.05).The blood glucose levels after the one-week nasal feeding and after two-week nasal feeding were significantly lower in the observation group and control group than in the first day of nasal feeding(P<0.05).But in the observation group the level of blood glucose in the first week of nasal feeding and the level of blood glucose in the two weeks of nasal feeding were significantly lower than in the control group(P<0.05).After 2 weeks of enteral nutrition,the incidence of vomiting and hyperglycemia in the observation group was significantly lower than that in the control group(P<0.05).There was no significant difference in the incidence of reflux and diarrhea between the two groups(P>0.05).Conclusion The modified nasal feeding method is helpful to slow down the deterioration of the nutritional status of ICU patients,and is beneficial to the control of blood glucose levels in ICU patients.Besides,the modified nasal feeding method is beneficial to reduce the occurrence of nasal feeding related complications.
