CTLA-4基因多态性与儿童支气管哮喘易感性及血浆IFN-γ、IL-4、IgE的关系

摘　　要：目的分析细胞毒性T淋巴细胞相关蛋白4(CTLA-4)基因rs4553808单核苷酸多态性与儿童哮喘易感性及血浆IFN-γ、IL-4、IgE的关系。方法选取112例哮喘患儿作为病例组,同期拟行择期手术的169例患儿作为对照组,应用聚合酶链反应(PCR)-限制性片段长度多态性(RFLP)方法检测两组血浆中CTLA-4基因rs4553808单核苷酸多态性,ELISA法检测血浆IFN-γ、IL-4、IgE,统计学分析CTLA-4基因rs4553808单核苷酸多态性与哮喘患儿血浆IFN-γ、IL-4、IgE水平及哮喘并发症的关系。结果病例组CTLA-4基因rs4553808 AA基因型比例及A等位基因频率高于对照组(P均<0.05),而GG基因型比例低于对照组(P<0.05)。CTLA-4基因rs4553808 AA基因型、GA基因型、A等位基因型儿童支气管哮喘的易感性增加(P均<0.05),CTLA-4基因rs4553808 AA基因型患儿血浆IFN-γ、IL-4、IgE水平高于GG型(P均<0.05)。CTLA-4基因rs4553808 AA基因型与消化道反应有关(P<0.05),而与纵隔或皮下气肿、呼吸衰竭、心率失常、肾炎等并发症无关(P均>0.05)。结论CTLA-4基因rs4553808 AA基因型及A等位基因可增加哮喘患儿的易感性,CTLA-4基因rs4553808 AA基因型与患儿血浆IFN-γ、IL-4、IgE水平有关。Objective To analyze the relationship between the single nucleotide polymorphism of cytotoxic T lymphocyte-associated protein 4(CTLA-4)gene rs4553808 and susceptibility to asthma in children as well as its correlation with the plasma levels of IFN-γ,IL-4,and IgE.Methods A total of 120 children with asthma were selected as the case group,and 169 children who were scheduled to undergo surgery were selected as the control group.Polymerase chain reaction(PCR)-restriction fragment length polymorphism(RFLP)was used to detect the single nucleotide polymorphism of CTLA-4 gene rs4553808 in the two groups.The plasma levels of IFN-γ,IL-4 and IgE were detected by ELISA.The relationships of CTLA-4 rs4553808 single nucleotide polymorphism with IFN-γ,IL-4,IgE levels and complications in asthma children were analyzed statistically.Results The proportion of CTLA-4 rs4553808 AA genotype and A allele frequency were significantly higher in the case group than in the control group,while the proportion of GG genotype was significantly lower(all P<0.05).The susceptibility of children with CTLA-4 rs4553808 AA genotype,GA genotype and A allele to asthma increased(all P<0.05).The plasma levels of IFN-γ,IL-4,and IgE in children with CTLA-4 rs4553808 AA genotype were significantly higher than those with GG genotype.CTLA-4 rs4553808 AA genotype was related to digestive tract reaction(P<0.05),but not to mediastinal or subcutaneous emphysema,respiratory failure,arrhythmia,or nephritis(all P>0.05).Conclusion CTLA-4 rs4553808 AA genotype and A allele increase the susceptibility to asthma in children,and CTLA-4 rs4553808 AA genotype is correlated with the plasma levels of IFN-γ,IL-4,and IgE.
