胰腺周围淋巴结结核的超声造影声像图特征及诊断价值

摘　　要：目的:探讨胰腺周围淋巴结结核的超声造影声像图特征及诊断价值。方法:回顾性分析2016年1月-2019年1月笔者所在医院31例胰腺周围淋巴结结核患者作为研究对象,均经过手术或穿刺病理组织活检证实。所有患者手术或活检前均接受超声造影和CT检查。分析胰腺周围淋巴结结核患者的超声造影图像,并比较超声造影图像和CT诊断胰腺周围淋巴结结核的准确率。结果:超声造影诊断胰腺周围淋巴结结核的准确率为90.32%(28/31),CT准确率为87.10%(27/31),二者比较差异无统计学意义(Х^2=0.010,P=0.922)。超声造影表现,Ⅰ型16例,呈环状增强型,病灶边缘可见造影剂填充迅速;Ⅱ型12例,呈不均匀增强型,其中边缘增强型6例,可表现为病灶周边出现高信号;分隔样增强型6例,可表现为病灶内出现条状增强。结论:超声造影诊断胰腺周围淋巴结结核具有一定价值。Objective:To investigate the characteristics and diagnostic value of contrast-enhanced ultrasonography(CEUS)in peripancreatic lymph node tuberculosis.Method:A total of 31 patients with peripancreatic lymph node tuberculosis in our hospital from January 2016 to January 2019 were analyzed retrospectively.All patients were confirmed by operation or biopsy.All patients underwent contrast-enhanced ultrasound and CT before operation or biopsy.To analyze the contrast-enhanced ultrasound images of patients with peripancreatic lymph node tuberculosis,and compare the accuracy of contrast-enhanced ultrasound images and CT diagnosis of peripancreatic lymph node tuberculosis.Result:The accuracy of contrast-enhanced ultrasonography and CT in diagnosis of peripancreatic lymph node tuberculosis was 90.32%(28/31)and 87.10%(27/31)respectively.There was no significant difference between the two methods(Х^2=0.010,P=0.922).Contrast-enhanced ultrasound manifestations were as follows:16 cases of typeⅠshowed circular enhancement with rapid filling of contrast media at the edge of the lesion.Twelve cases of typeⅡshowed heterogeneous enhancement,including 6 cases of marginal enhancement,which could be characterized by high signal around the lesion,and 6 cases of septal enhancement,which could be characterized by strip enhancement in the lesion.Conclusion:Contrast-enhanced ultrasonography is valuable in the diagnosis of peripancreatic lymph node tuberculosis.
