高血压和高血压合并室性心律失常患者的心率变异性分析

摘　　要：目的:探讨高血压和高血压合并室性心律失常(VA)患者的心率变异性。方法:选取2017年6月-2019年3月在笔者所在医院就诊的高血压合并VA患者40例,根据动态心电图显示,<lown3级的为B1组(20例),≥lown3级的为B2组(20例),选取同时期无VA的高血压患者20例为A组。对比三组HRV时域指标、频域指标及室性心律失常的发生与HRV时域和频域指标的相关性。结果:B1组、B2组SDANN、SDNN、MSSD和PNN50水平均低于A组,且B2组SDANN和SDNN水平均低于B1组,差异均有统计学意义(P<0.05);B1组、B2组LVF、HF和LF水平均低于A组,LF/HF比值高于A组,差异均有统计学意义(P<0.05);SDANN、SDNN和PNN50与室性心律失常的发生呈负相关(P<0.05)。结论:高血压合并VA患者的SDANN、SDNN和PNN50与室性VA的发生呈负相关,这可以为临床筛查和治疗提供有力的客观证据。Objective:To investigate heart rate variability in patients with hypertension and hypertension complicated with ventricular arrhythmia(VA).Method:Forty patients with hypertension and VA who were admitted to our hospital from June 2017 to March 2019 were selected.According to the dynamic electrocardiogram,<lown 3 was group B1(20 cases),≥lown 3 was group B2(20 cases).A total of 20 patients with hypertension without VA at the same time were selected as group A.The HRV time domain indicators,frequency domain indicators and the occurrence of ventricular arrhythmia and HRV time domain and frequency domain indicators among were compared.Result:The levels of SDANN,SDNN,MSSD and PNN50 in group B1 and group B2 were lower than those in group A,and the levels of SDANN and SDNN in group B2 were lower than those in group B1,the differences were statistically significant(P<0.05).The levels of LVF,HF and LF in group B1 and group B2 were lower than those in group A,the ratio of LF/HF in group B1 and group B2 was significantly higher than that in group A,the differences were statistically significant(P<0.05).The SDANN,SDNN and PNN50 were negatively correlated with the occurrence of ventricular arrhythmia(P<0.05).Conclusion:SDANN,SDNN,and PNN50 in patients with hypertension and VA are inversely associated with the occurrence of ventricular VA,which provides strong objective evidence for clinical screening and treatment.
