摘 要:目的探讨胫骨高位截骨术治疗膝关节内侧间室骨关节炎的效果及对血清炎性因子、PⅡANP水平的影响。方法选取2017年4月至2018年6月本院收治的55例膝关节内侧间室骨关节炎患者为研究对象,所有患者均行胫骨高位截骨术治疗。观察患者的治疗效果。结果术后3 d、1个月的HSS评分与术前比较,差异无统计学意义(P>0.05);术后3、6个月和1年的HSS评分明显高于术前,差异具有统计学意义(P<0.05);术后3 d、1、3、6个月及1年的VAS评分均低于术前,差异具有统计学意义(P<0.05)。术后不同时间的WOMAC评分均较术前升高,差异具有统计学意义(P<0.05);术后不同时间的胫骨近端内侧角及生理性外翻角均较术前显著增大,差异具有统计学意义(P<0.05)。术后不同时间的IL-6、IL-17、IL-1和PⅡANP水平均显著低于术前,差异具有统计学意义(P<0.05)。结论胫骨高位截骨术治疗膝关节内侧间室骨关节炎能有效改善患者膝关节功能,减轻疼痛症状,消除血清炎症因子,具有推广应用价值。Objective To investigate the effect of high tibial osteotomy in the treatment of medial compartment osteoarthritis of the knee joint and its influences on serum inflammatory factors and P ⅡANP level. Methods A total of 55 patients with medial compartment osteoarthritis of the knee join admitted in our hospital from April 2017 to June 2018 were selected as the study objects, all of them received high tibial osteotomy. The therapeatic effect of the patients were observed. Results There were no significant differences in HSS scores of 3 days and 1 month after operation compared with that before operation(P>0.05);HSS scores at 3, 6 months and 1 year after operation were significantly higher than those before operation, and the differences were statistically significant(P<0.05);VAS scores at 3 days, 1, 3, 6 months and 1 year after operation were all lower than those before operation, and the differences were statistically significant(P<0.05).The score of WOMAC at different time after operation were higher than those before operation, and the differences were statistically significant(P<0.05);the medial angle of proximal tibia and physiological valgus at different time after operation increased significantly compared with those before operation, and the differences were statistically significant(P<0.05).The levels of IL-6, IL-17, IL-1 and P ⅡANP at different time after operation were significantly lower than those before operation, and the differences were statistically significant(P<0.05). Conclusion High tibial osteotomy for the treatment of medial compartment osteoarthritis of knee joint can effectively improve knee joint function of patients, relieve pain symptoms, and eliminate serum inflammatory factors. It has the value of popularization and application.
