摘  要:目的:探讨介入下置管引流在治疗重症胰腺炎局部并发症的可行性。方法:回顾性分析2017年1月-2019年6月的30例胰腺炎患者,经CT确定穿刺路径后,在超声或CT引导下置管引流治疗急性液体积聚、胰腺组织坏死、胰腺周围脓肿等并发症。观察患者引流期间的临床表现,相关炎症反应指标TNF、IL-6、CRP在引流前后的变化。结果:30例患者穿刺置管引流全部成功,引流后临床症状改善,TNF、IL-6、CRP在引流后的明显下降,差异有统计学意义(P<0.05)。最终2例患者接受了手术治疗,1例死亡。结论:遵循微创化的治疗原则,选择性应用超声或CT引导下置管引流治疗胰腺炎局部并发症是可行的安全和有效的方法,值得推广应用。Objective:To investigate the feasibility of percutaneous catheter drainage by interventional way on complications for the treatment of severe acute pancreatitis(SAP).Method:From January 2017 to June 2019 the clinical data of 30 SAP patients were retrospectively analyzed.Under the guidance of ultrasound or CT,percutaneous catheter drainage was used to deal with fluid collection,pancreatic necrosis,peripancreatic abscess and other complications.The clinical symptoms of patients were observed during drainage,inflammatory factors such as TNF,IL-6 and CRP were also observed.Result:Percutaneous catheter drainage was successfully accomplished in all 30 cases,and no puncture complication happened.Clinical symptoms of patients were improved.The levels of inflammatory factor such as TNF,IL-6 and CRP were respectively lower than before drainage(P<0.05).Two patients underwent operations,1 case died.Conclusion:According to the principle of minimal invasion,the selective application of percutaneous catheter drainage guided by ultrasound or CT to deal with the complications in SAP is not only safe but also effective,and is worthy of popularization and application.
