摘 要:目的探讨显微手术在手外伤患者中的应用效果。方法选取2017年1月至2018年10月我科室收治的50例手外伤的患者为研究对象,按照治疗方式的不同将其分为对照组(25例,常规手术治疗)与观察组(25例,显微手术治疗)。比较两组患者的手康复情况、一般手术情况、疼痛评分、组织病理学评分、瘙痒评分及生活质量评分。结果观察组患者的手康复总有效率明显高于对照组(P<0.05)。观察组患者的手术时间长于对照组,术中出血量少于对照组,创面愈合时间及住院时间短于对照组(P<0.05);两组患者的并发症总发生率比较,差异无统计学意义(P>0.05)。治疗后,两组患者的疼痛评分及组织病理学评分均改善,且观察组优于对照组(P<0.05);治疗后,观察组患者的瘙痒评分明显低于对照组(P<0.05)。治疗后,两组患者的GQOLI-74评分均升高,且观察组高于对照组,差异均有统计学意义(P<0.05)。结论显微手术在手外伤患者中的应用效果显著,可改善患者的手外伤恢复情况,减少患者的术中出血量,缩短患者的创面愈合时间及住院时间,降低患者的疼痛及瘙痒程度,提高患者的生活质量,安全性高,值得临床推广与应用。Objective To explore the application effect of microsurgery in the treatment of patients with hand injury.Methods Fifty patients with hand injury admitted in our department from January 2017 to October 2018 were selected as the research objects, and the patients were divided into control group(25 cases, routine surgical treatment) and observation group(25 cases, microsurgery treatment) according to different treatment methods. The hand rehabilitation,general operation, scores of pain, histopathology, pruritus and quality of life of the two groups were compared. Results The total effective rate of hand rehabilitation in the observation group was significantly higher than that in the control group(P <0.05). The operation time of the observation group was longer than that of the control group, the amount of bleeding during operation was less than that of the control group, and the wound healing time and hospitalization time were shorter than those of the control group(P <0.05);there was no significant difference in the total incidence of complications between the two groups(P>0.05). After treatment, the pain scores and histopathological scores of the two groups improved significantly, and those in the observation group were better than the control group( P <0.05);after treatment, the pruritus score of the observation group was significantly lower than that of the control group( P<0.05). After treatment, the GQOLI-74 scores of the two groups increased significantly, and those in the observation group were higher than the control group, the differences were statistically significant(P <0.05). Conclusion Microsurgery treatment has a significant effect in patients with hand injury, it can improve the recovery of hand injury, reduce intraoperative bleeding,shorten wound healing time and hospitalization time, reduce pain and itching, improve the quality of life of patients, and has high safety, which is worthy of clinical promotion and application.
