摘　　要：目的探讨鼻内镜下上颌窦窦口开放联合下鼻道开窗入路手术治疗真菌性鼻窦炎的有效性和安全性。方法选取2013年9月至2018年8月于我院行手术治疗的真菌性鼻窦炎患者146例,依患者意愿分为对照组(n=69)和研究组(n=77),分别行鼻内镜下上颌窦窦口开放术和鼻内镜下上颌窦窦口开放联合下鼻道开窗入路术治疗,随访3个月。比较两组手术前及随访结束后的鼻内镜评分、生存质量、术后并发症发生情况。结果随访结束后,两组患者的鼻内镜评分和生存质量评分均明显低于手术前,且研究组低于对照组(P<0.05)。两组患者术后并发症总发生率比较,差异无统计学意义(P>0.05)。结论鼻内镜下上颌窦窦口开放联合下鼻道开窗入路术治疗真菌性鼻窦炎可改善患者的临床症状,提高其生存质量,且并发症少。Objective To discuss the efficacy and safety of endoscopic maxillary ostium surgery combined with inferior meatus fenestration in the treatment of fungal sinusitis. Methods One hundred and forty-six patients with fungal sinusitis who underwent surgical treatment in our hospital from September 2013 to August 2018 were selected and divided into control group(n =69) and study group(n =77) according to the patients’ wishes. The patients were respectively treated with endoscopic maxillary ostium surgery and endoscopic maxillary ostium surgery combined with inferior meatus fenestration and followed up for 3 months. The nasal endoscopic score, quality of life score before surgery and at the end of follow-up and postoperative complications of the two groups were compared. Results At the end of follow-up, the scores of nasal endoscopy and quality of life of the two groups were significantly lower than those before treatment, and those of the study group were lower than the control group(P<0.05). There was no significant difference in the total incidence of postoperative complications between the two groups(P>0.05). Conclusion Endoscopic maxillary ostium surgery combined with inferior meatus fenestration in the treatment of fungal sinusitis can improve the clinical symptoms, and enhance the life quality of patients, with few complications.
