三种治疗腋臭方法的效果比较分析

摘　　要：目的比较小切口皮下搔刮术、CO2激光和A型肉毒毒素注射3种治疗腋臭方法的临床效果。方法回顾性分析417例腋臭患者的临床资料,根据采用的治疗方法分为小切口皮下搔刮术组(144例)、CO2激光组(141例)和A型肉毒毒素组(132例)。评估术后气味、出汗及生活质量的改善程度,满意度,临床改善率,并发症及复发情况。结果 3种治疗方式均能显著改善患者的气味、出汗程度和生活质量(P<0.05);术后12个月,A型肉毒毒素组临床改善率和满意度明显低于其他两组(P<0.05);与其他两组相比,小切口皮下搔刮术组的复发间隔时间最长(P<0.05),A型肉毒毒素组复发率较高(P<0.05);小切口皮下搔刮术组的并发症发生率高于其他两组。结论 3种腋臭治疗方法中,小切口皮下搔刮术是较佳的手术治疗方法;而非手术治疗方法中CO2激光治疗效果较好。Objective To compare the therapeutic effects of subcutaneous curettage through small incision, carbon dioxide laser and botulinum toxin A injection in the treatment of axillary osmidrosis. Methods According to the treatment method, 417 patients were divided into group of subcutaneous curettage through small incision(144 cases), group of carbon dioxide laser(141 cases) and group of botulinum toxin A(132 cases). The smell, sweating condition, improvement degree of quality of life, degree of satisfaction, clinical improvement rate, complications and recurrence were assessed. Results The smell, sweating condition and quality of life were significantly improved in three groups(P<0.05). Twelve months after treatment, the clinical improvement rate and degree of satisfaction of group of botulinum toxin A injection were significantly lower than those of the other two groups(P<0.05). Compared with patients receiving other treatments, patients receiving small incision subcutaneous curettage had the longest. The recurrence interval was the longest in the group of subcutaneous curettage through small incision(P<0.05), and the recurrence rate of group of botulinum toxin A injection was higher than that of the other two groups(P<0.05). The incidence rate of complications of group of subcutaneous curettage through small incision was higher than that of the other two groups. Conclusion The subcutaneous curettage through small incision was a good method for the treatment of axillary osmidrosis. The therapeutic effect of carbon dioxide laser was better than botulinum toxin A injection in the non-operative treatment.
