中药蜡疗联合常规疗法与护理措施干预早中期膝关节骨性关节炎临床研究

摘　　要：目的:观察在常规治疗与护理的基础上给予中药蜡疗干预早中期膝关节骨性关节炎的疗效。方法:选取80例瘀血痹阻型早中期膝关节骨性关节炎患者,随机将其分为观察组与对照组各40例。2组均予常规治疗与护理,观察组则加予中药蜡疗,2组均治疗4周。治疗前后采用症状及体征观察量表评价临床症状及体征的改善情况,采用Rasmussen膝关节功能评分法评价功能障碍严重程度,比较2组的临床疗效。结果:治疗后,临床疗效总有效率观察组95.0%,对照组60.0%,2组比较,差异有统计学意义(P<0.05)。2组麻木、疼痛、行走、直腿抬高试验评分与膝腱反射、跟腱反射评分以及总积分均较治疗前升高(P<0.05),观察组各项评分和总积分均高于对照组(P<0.05)。2组Rasmussen膝关节功能评分法的功能部分、疼痛、稳定性、活动范围、屈曲畸形、肌力部分评分均较治疗前升高(P<0.05),观察组各项评分均高于对照组(P<0.05)。结论:在常规治疗与护理的基础上给予中药蜡疗干预早中期膝关节骨性关节炎患者,能有效缓解膝关节的疼痛与临床症状,促进关节功能恢复,对提高患者的生活质量具有重要的作用。Objective:To observe the curative effect of intervention of wax therapy with Chinese herbal medicine based on routine treatment and nursing for osteoarthritis of the knee in the early and middle stages.Methods:A total of 80 cases of patients with osteoarthritis of the knee of static blood obstruction type in the early and middle stages were randomly divided into the observation group and the control group,40 cases in each group.Both groups were given routine treatment and nursing,and the observation group was additionally treated with wax therapy with Chinese herbal medicine.Both groups were treated for four weeks.Before and after treatment,the improvement of clinical symptoms and signs was evaluated by the Observation Scale for Symptom and Sign,and the degree of dysfunction was evaluated by Rasmussen Knee Functional Score.The clinical effect in the two groups was compared.Results:After treatment,the total clinical effective rate was 95.0%in the observation group,and 60.0%in the control group,the difference being significant(P<0.05).The scores of numbness,pain,walking,straight leg rising test,patellar reflex and Achilles reflex and the total scores in the two groups were increased when compared with those before treatment(P<0.05),and the above scores in the observation group were all higher than those in the control group(P<0.05).The scores of knee function,pain,stability,range of motion,flexion deformity and muscle strength in the Rasmussen Knee Functional Score in the two groups were increased when compared with those before treatment(P<0.05),and each score in the observation group was higher than that in the control group(P<0.05).Conclusion:Based on routine treatment and nursing,the intervention of wax therapy with Chinese herbal medicine for patients with osteoarthritis of the knee in the early and middle stages can effectively relieve the pain of knee and clinical symptoms,promote the recovery of joint function,and play an important role in improving the quality of life.
