摘 要:目的探讨高级别鳞状上皮内病变(HSIL)的宫颈血管三维能量多普勒超声(3DPDUS)特征及与血管内皮生长因子(VEGF)、微血管密度(MVD)的关系。方法收集怀疑宫颈病变者160例,根据病理结果分为非宫颈上皮内瘤变(CIN)组34例、低级别病变(LSIL)46例与HSIL组79例,另1例宫颈癌。分析比较宫颈常规彩色多普勒超声(CDFI)参数和三维血管参数,并检测病变组织的VEGF表达及MVD情况。结果 HSIL组的RI显著低于LSIL组和非CIN组,PSV、FI、VI、VFI及组织中VEGF表达、MVD均显著高于LSIL组和非CIN组(P<0.05)。HSIL组的MVD、VEGF表达与RI呈显著负相关性(P<0.05),与FI、VI、VFI呈显著正相关性(P<0.05),但与PSV无明显相关性(P>0.05)。结论 HSIL具有宫颈新生血管和血流信号异常且与VEGF、MVD密切相关,3DPDUS能够反映HSIL组织的新生血管情况,对HSIL的早期筛查与诊疗具有指导价值。Objective To investigate the characteristics of three-dimensional power Doppler ultrasound(3 DPDUS)of cervical vascular and its relationship with microvascular density(MVD),vascular endothelial growth factor(VEGF)in patients with cervical high-grade squamous intraepithelial lesion(HSIL).Methods 160 patients with suspected cervical lesions were divided into 34 cases of non-cervical intraepithelial neoplasia(CIN)group,79 cases of low-grade lesions(LSIL),79 cases of HSIL group and 1 patient with cervical cancer according to pathological results.The routine cervical color Doppler ultrasound(CDFI)parameters and three-dimensional vascular parameters were compared and analyzed.The VEGF expression and MVD in tissues was detected.Results The RI of the HSIL group was significantly lower than that of the LSIL group and the non-CIN group,while PSV,FI,VI,VFI and VEGF,MVD of the tissues were significantly higher than those of the LSIL group and the non-CIN group(P<0.05).The MVD and expression of VEGF in HSIL group was significantly negatively correlated with RI(P<0.05),and positively correlated with FI,VI and VFI(P<0.05),but had no significant correlation with PSV(P>0.05).Conclusions HSIL has abnormal cervix neovascularization and blood flow signals,and they are closely related to VEGF and MVD.3 DPDUS mar reflect the condition of cervical vascularization of HSIL,which can provide effective help for the early screening,diagnosis and treatment of HSIL.
