评价不同气管插管方式在ICU严重呼吸衰竭患者治疗中的应用

摘　　要：目的比较经口腔和经鼻腔气管插管在ICU严重呼吸衰竭(RF)治疗中的应用。方法研究时间以2018年8月至2019年8月为准,对此期间内ICU接诊的130例RF病人为观察对象。根据随机方法分组,A组经口气管插管,B组经鼻气管插管。对比两组插管情况、置管期间并发症情况、病人满意度。结果A组病人插管时间(48.42±5.12)s、留置管时间(5.03±1.24)d均比B组(60.15±5.88)s、(8.75±2.02)d更短,P<0.05。两组病人置管期间总并发症发生率相比,并无明显差异(P>0.05);但A组意外拔管、口腔溃疡等并发症明显高于B组,P<0.05。两组病人对通气治疗的满意度均无明显差异[92.31%vs95.38%](P>0.05)。结论对于呼吸衰竭病人而言,经口插管和经鼻插管都有各自的优势,实际应用中要根据病人的情况选择。Objective to compare application of transoral and transnasal endotracheal intubation in treatment of severe respiratory failure(RF)in ICU.Methods treat 130 cases RF patients in ICU from August 2018 to August 2019 as observation objects.Divide them into group with random method,group A was intubated transorally,and group B intubated transnasally.Compare intubation situation,complications and satisfaction degree of patients between two groups.Results intubation time(48.42±5.12)s and indwelling time(5.03±1.24)d in group A was shorter than group B(60.15±5.88)s and(8.75±2.02)d(P<0.05).Total complications incidence during catheterization no significant difference between two groups,with no significant difference,(P>0.05),but complications of accidental extubation and oral ulcer in group A was significantly higher than group B(P<0.05).Satisfaction degree with ventilation treatment showed no significant difference between two groups[92.31%vs 95.38%](P>0.05).Conclusion transoral and transnasal intubation has their own advantages for respiratory failure patients,which should be selected according to actual situation of patients.
