摘　　要：以总院为核心,以社区医院为桥梁,通过医联体模式,从总院、分院、社区患者3个层面挖掘医联体内VTE防治现状水平与攻坚点,多方拟定方策并反复循证,结合现实需求,搭建血栓风险评估网络平台,构建医联体VTE规范化防治流程,提高患者自我抗栓意识及依从性,构建了医联体协作VTE防治模式,更好地确保了患者安全。Institute as the core,community hospitals as a bridge,with medical alliance mode,the current situation and crucial points of VTE prevention and treatment were analyzed from three levels of general hospital,branch,and community patients.Multiple prescriptions were formulated with repeatedly evidence-based practice.Combined with the actual needs,a network platform for thrombus risk assessment was built,and standardized prevention and treatment process of medical alliance VTE was constructed.Patients'self-anti-thrombotic awareness and compliance were improved,and the cooperative VTE prevention and treatment model of medical alliance was constructed,which better ensured the safety of patients.
