基于“形寒寒饮伤肺”理论的慢性阻塞性肺疾病内外合治诊疗方案的随机对照研究

摘　　要：目的基于"形寒寒饮伤肺"理论,提出慢性阻塞性肺疾病核心病机"寒饮伏肺",中医辨证治疗应"不远温",制定了内外合治的特色诊疗方案并验证疗效。方法选择慢性阻塞性肺疾病160例,采用随机对照方法分为研究组及对照组,研究组选用内外合治诊疗方案,对照组选用西医常规治疗方法。结果2组中医证候疗效比较,研究组有效率为93.42%,对照组有效率为87.84%,组间比较差异具有统计学意义(P<0.05),研究组在改善肺功能(FEV1、FEV1占预计值%、FEV1/FVC)、增加6分钟步行距离等方面优于对照组(P<0.05)。结论内外合治诊疗方案,对改善慢性阻塞性肺疾病患者中医证候及肺功能等均有一定疗效。Objective Based on the theory of"the lung damage due to cold body and cold excessive fluid",the head-doctor CAI Hongyan proposed that the core pathogenesis of chronic obstructive pulmonary disease is"cold excessive fluid retention in lung"and treatment by differentiation of syndromes should be"not far from warm drugs",and developed the characteristic treatment plan combining TCM internal and external treatment and also verified its clinical effect.Methods 160 patients were selected and were randomly divided into treatment group and control group.The patients in treatment group were taken the treatment plan combining TCM internal and external treatment and the patients in control group treated with routine western medicine.Results Comparing the TCM syndromes effect between the treatment group and the control group,the effective rate of the treatment group was 93.42%,and the effective rate of the control group was 87.66%.The difference between the groups was statistically significant(P<0.05).The treatment group were better than the control group in improving lung function and 6 MWD(P<0.05).Conclusion The combination of internal and external treatment has certain curative effect on improving TCM syndrome and lung function of COPD patients,and has further research value.
