摘  要:目的分析64层螺旋CT多方位重组技术(MPR)与纤维支气管镜(FOB)对支气管结核(EBTB)的诊断准确性及二者对肺功能的影响。方法100例我院收治的支气管结核患者作为观察对象,所有患者经实验室和影像学检测确诊为EBTB,患者分别接受MPR技术(MPR实验组)和FOB(FOB组)检查,比较不同检查方法在EBTB诊断中的价值,并分析2种检查方法对肺功能的影响。结果MPR组诊断符合率为95.0%(95/100),FOB组诊断符合率为97.0%(97/100),2组差异无统计学意义(P>0.05);MPR和FOB组对受累部位的检出数量分别比较,差异无统计学意义(P>0.05);FOB组检查后肺活量、第1秒用力呼吸容量及最大自主通气量均显著低于检查前(P<0.05),但MPR组检查前后肺功能无显著变化(P>0.05)。结论64层螺旋CT扫描后MPR技术与FOB技术对EBTB的诊断均有较高的准确率,但FOB检查影响患者肺功能。Objective To investigate the accuracy of 64 slice spiral CT multi plate reconstruction technique(MPR)and fiberoptic bronchoscopy(FOB)in diagnosis of bronchial tuberculosis diagnosis and their effects on lung function of patients.Methods A total of 100 patients with bronchial tuberculosis who were admitted and treated in our hospital were enrolled in the stydy,who underwent MPR check and fiberoptic bronchoscopy examination.The diagnostic accuracy rate,and the effects of both examination methods on lung function including vital capacity,forced expiratory volume in first second(FEV1)and maximum spontaneous ventilation were observed and analyzed.Results There was no significant difference in the diagnostic accuracy rate between MPR group and FOB group(95.0%vs 97.0%,P>0.05).Moreover,there were no significant differences in the detective numbers of affected sites between MPR group and FOB group(P>0.05).And the vital capacity,FEV1 and maximum spontaneous ventilation after examination by FOB were significantly lower than those before examination(P<0.05),however,there were no significant differences in lung function indexes detected by MPR before and after examination(P>0.05).Conclusion Both MPR technology and FOB have higher accuracy rate in diagnosis of bronchial tuberculosis,however,MPR technology has effects on lung function of patients.
