摘  要:目的探索血浆肝素结合蛋白(Heparin-Binding Protein,HBP)与外周血CD64对慢性阻塞性肺疾病急性加重期患者细菌感染病原菌的关系。方法回顾性分析2014年5月-2017年6月于宁波市医疗中心李惠利医院接受治疗的98例慢性阻塞性肺疾病急性加重期患者的临床资料,按照痰细菌培养结果将患者分为感染组(n=64)和无感染组(n=34)。检测并比较两组患者的HBP和CD64指数,比较感染组不同类型感染细菌者的HBP和CD64指数。结果感染组患者的HBP为(87.24±26.57)ng/ml,CD64指数为(6.38±2.15),均高于无感染组患者,差异有统计学意义(P<0.05)。感染组共64例患者,其中感染类型为革兰阳性菌的患者27例,感染革兰阴性菌的患者37例。感染革兰阳性菌患者的HBP为(99.65±11.28)ng/ml,CD64指数为(7.82±2.54),均高于感染革兰阴性菌患者,差异有统计学意义(P<0.05)。结论HBP和CD64能够作为细菌感染早期鉴别的重要诊断指标,同时HBP还能够作为细菌感染类型鉴别的指标,对于指导患者的临床用药和早期治疗具有重要意义。OBJECTIVE To explore the correlation between plasma heparin-binding protein(HBP)and peripheral blood CD64 and pathogens of bacterial infection in patients with acute exacerbation of chronic obstructive pulmonary disease.METHODS Retrospective analysis of clinical data of 98 patients with acute exacerbation of chronic obstructive pulmonary disease in Lihuili Hospital of Ningbo Medical Center from May 2014 to June 2017.Patients were divided into infected groups(n=64)and non-infected group(n=34)according to the results of sputum bacterial culture.The HBP and CD64 indices in the two groups and different types of infected bacteria in the infected group were compared.RESULTS The HBP(87.24±26.57)ng/ml and CD64 index(6.38±2.15)in the infected group were significantly higher than that in the non-infected group and the control group(P<0.05).There were 64 patients in the infected group,including 27 patients with Gram-positive bacteria and 37 patients with Gram-negative bacteria.The concentration of HBP(99.65±11.28)ng/ml and CD64 index(7.82±2.54)were significantly higher in patients infected with Gram-positive bacteria than in patients infected with Gram-negative bacteria(P<0.05).CONCLUSION HBP and CD64 can be used as important diagnostic indicators for the early identification of bacterial infections.At the same time,HBP can also be used as an indicator for the identification of bacterial infection types,which is of great significance for guiding patients’clinical medication and early treatment.
