摘　　要：目的了解云南省15岁及以上居民烟草使用及二手烟暴露状况,为评价云南省成人烟草流行状况及制定相应的控烟政策提供依据。方法于2017年采用分层多阶段整群概率抽样法抽取全省6 369名15岁及以上常住居民为研究对象,使用"中国成人烟草调查问卷"进行面对面问卷调查,问卷内容包括基本情况、烟草使用、戒烟和二手烟暴露等信息。采用SPSS 22.0统计软件的复杂抽样分析程序进行χ^2检验。结果 2017年云南省15岁及以上居民现在吸烟率为32.2%(95%CI:30.4%~34.1%),其中男性为62.1%(95%CI:59.4%~64.8%),女性为1.2%(95%CI:0.7%~2.0%);城市人群为28.9%(95%CI:26.1%~31.9%),农村人群为34.9%(95%CI:32.7%~37.1%)。云南省15岁以上居民每周二手烟暴露率为82.8%(95%CI:79.8%~85.5%),其中男性为88.5%(95%CI:84.8%~91.4%),女性为76.9%(95%CI:73.4%~80.0%);吸烟者为94.2%(95%CI:91.4%~96.2%),非吸烟者为77.3%(95%CI:73.6%~80.6%)。云南省15岁以上居民戒烟率为13.6%(95%CI:11.7%~15.7%),戒烟尝试的比例为12.8%(95%CI:11.0%~14.9%),有戒烟意愿的比例为13.8%(95%CI:9.8%~19.4%)。结论云南省烟草流行形势严峻,现在吸烟率及二手烟暴露率较高,亟待出台地方性控烟立法,降低人群吸烟率,减少公共场所二手烟暴露。Objective To understand the tobacco use and secondhand smoking exposure situation in residents (≥15 years old) of Yunnan Province, and to provide the basis for evaluating the tobacco prevalence situation among adults, as well as formulating corresponding tobacco control policies in Yunnan Province. Methods In 2017, the stratified multi-stage cluster probability sampling method was used to select 6 369 residents (≥15 years old) as the subjects. The investigation was performed with "Chinese adult tobacco questionnaire" which included the basic condition,tobacco usage,the second hand smoking exposure and smoking cessation. The complex sampling analysis program was used to analyze the data by χ^2 test, the used software was SPSS22.0. Results The present smoking rate in residents (≥15 years old) was 32.2%(95% CI:30.4%-34.1%;male: 62.1%,95%CI:59.4%-64.8%;female: 1.2%,95% CI:0.7%-2.0%);the present smoking rates of urban residents and rural residents were28.9%(95%CI:26.1%-31.9%) and 34.9%(95%CI:32.7%-37.1%). The rate of the weekly second hand smoking exposure in residents (≥ 15 years old) was 82.8%(95%CI:79.8%-85.5%;male: 88.5%,95%CI:84.8%~91.4%;female:76.9%, 95%CI:73.4%~ 80.0%). The rates of the weekly second hand smoking exposure in smokers and non-smokers were 94.2%(95%CI:91.4%-96.2%) and 77.3%(95%CI:73.6%-80.6%). The smoking cessation rate of residents (≥15 years old) in Yunnan Province was13.6%(95% CI:11.7%-15.7%),the cessation attempting rate of smokers was 12.8%(95% CI:11.0%-14.9%),and 13.8%(95%CI:9.8%-19.4%) of smokers have the willing to quit smoking. Conclusion The situation of tobacco prevalence in Yunnan Province is serious, the smoking rate and the secondhand smoke exposure rate are higher. It is urgent for Yunnan Province to publish local tobacco control legislation to reduce the smoking rate and the exposure of secondhand smoking in public places.
