摘 要:目的了解扬州市职业暴露人群H5N1禽流感病毒抗体水平和外环境中禽流感病毒的污染状况.方法在2012—2018年采集扬州市禽流感职业暴露人群血清标本采用血凝抑制试验检测H5N1抗体,采集活禽市场等外环境标本采用实时荧光PCR法检测甲型禽流感病毒(FluA)、H5亚型、H7亚型、H9亚型核酸.结果964份职业暴露人群血清中,H5N1抗体均为阴性.2493份外环境标本中,共检出FluA阳性标本209份,总阳性率为8.38%,其中H5、H7、H9亚型阳性构成比分别为3.83%、0.96%、54.55%,混合感染占40.19%,且不同年度、不同地区、不同类型监测场所、不同类型标本及不同月份阳性检出率差异均有统计学意义(P<0.05).结论扬州市职业暴露人群血清标本中未检出H5N1抗体阳性标本,但外环境中常年存在H5、H7、H9亚型禽流感病毒的污染,发生风险主要集中在冬春季节和城乡活禽市场.Objective To understand the antibody levels of H5N1 avian influenza virus(AIV)among occupational exposure population and the contamination status of AIV in the external environment in Yangzhou city.Methods The serum samples were collected from occupational exposure population of H5N1 AIV in Yangzhou from 2012 to 2018.Antibody against H5N1 AIV was detected by hemagglutination inhibition test.External environmental samples were collected from live poultry markets and other sites.Nucleic acids of influenza A virus(Flu A),subtypes of H5,H7 and H9 were detected by real-time fluorescent PCR.Results All 964 serum samples from occupational exposure population were negative for antibody against H5N1 AIV.For 2493 external environment samples,209 samples were positive for Flu A with an overall positive rate of 8.38%.The constitutional ratios of positive results for H5,H7,H9 and mixed infections were 3.83%,0.96%,54.55%and 40.19%,respectively.There were statistically significant differences(P<0.05)among positive rates of different years,regions,surveillance sites,specimen types and months.Conclusions No antibody-positive sample of H5N1 AIV was found in occupational exposure population. Contamination of H5, H7 and H9 AIV distributed widely in the surveillance areas in Yangzhou.The high risk of AIV infection existed mainly in urban and rural live poultry markets and in winter-spring seasons.
