摘 要:目的探究高压氧治疗对颅内动脉瘤破裂蛛网膜下腔出血(aSHA)术后患者脑功能恢复的影响效果。方法选择2015年3月到2018年5月在我院神经外科确诊颅内动脉瘤破裂蛛网膜下腔出血(aSHA)并行血管内介入治疗术的患者68例,以术后是否高压氧治疗将其分为治疗组和对照组,各34例,对照组术后行常规治疗,治疗组术后在常规治疗基础下行高压氧治疗,对两组性别、年龄、改良Fisher分级、大脑中动脉(MCA)血液平均流速、脑血管痉挛(CVS)、脑水肿评分、神经功能缺损(NFD)评分、格拉斯哥预后(GOS)、格拉斯哥昏迷量表(GCS)及总不良反应进行比较。结果治疗后,治疗组改良Fisher分级、MCA血液平均流速、CVS、脑水肿评分、NFD评分及GCS评分优于较对照组,有统计学差异(P<0.05);治疗后,GOS评分V级恢复良好状态治疗组6例(17.65%)高于对照组25例(73.53%)(P<0.05);治疗后,对照组总不良反应高于治疗组(P<0.05)。结论高压氧治疗可显著改善脑神经功能缺损,提高脑动脉血流速度,促进aSHA术后患者脑功能恢复,使不良反应减少,提高患者日常活动能力。Objective To investigate the effect of hyperbaric oxygen therapy on cerebral functional rehabilitation of postoperative patients with aneurysmal subarachnoid hemorrhage(aSAH).Methods 68 aSAH patients underwent endovascular intervention in our hospital from March 2015 to May 2018 were enrolled and divided into treatment group and control group according to the presence or absence of the postoperative hyperbaric oxygen therapy,each with 34 cases.The control group received conventional treatment after surgery,and the treatment group received hyperbaric oxygen treatment combined with conventional treatment after surgery.Then the gender,age,modified Fisher scale,mean blood flow velocity of middle cerebral artery(MCA),cerebral vasospasm(CVS),brain edema score,Neurological Function Deficits scale(NFD)score,Glasgow outcome scale(GOS),Glasgow coma scale(GCS)and total adverse reactions were compared between the two groups.Results After treatment,the modified Fisher scale,post-treatment MCA blood flow rate,CVS,brain edema score,NFD score and GCS score of the treatment group was better than that of the control group with statistical difference(P<0.05).The ratio of GOS score at V grade in the treatment group was higher than that of the control group[17.65%(6 cases)VS 73.53%(25 cases)](P<0.05).The total incidence rate of the adverse reactions in the control group were higher than that in the treatment group(P<0.05).Conclusion Hyperbaric oxygen therapy can significantly ameliorate the brain nerve function defect,increase the cerebral artery blood flow velocity,promote the recovery of brain function,reduce adverse reactions and improve the daily activity ability of aSAH patients.
