摘　　要：目的探讨阻塞性睡眠呼吸暂停低通气综合征外周血miR-223、MMP-9的表达水平及意义。方法选取2017年1月至2018年12月在我院收治的OSAHS患者50例作为观察组,以同期健康体检的健康者50例作为对照组。采用睡眠呼吸暂停低通气指数(AHI)对疾病组患者病情程度进行划分,其中轻度17例(AHI≤10次/h)、中度22例(10次/h>AHI≤20次/h)、重度11例(AHI>20次/h)。采用荧光定量PCR法检测受试者外周血miR-223水平,采用酶联免疫吸附法(ELISA)检测受试者外周血MMP-9水平。观察两组外周血的miR-223、MMP-9表达水平差异,并比较两组之间的差异。分析OSAHS不同病情程度的miR-223、MMP-9表达水平,应用ROC曲线分析miR-223、MMP-9指标在预测OSAHS病情程度的效能。结果观察组患者的外周血miR-223相对表达量明显低于对照组,而MMP-9表达水平明显高于对照组,差异有统计学意义(P<0.05);OSAHS患者病情轻度、中度及重度的miR-223相对表达量及MMP-9表达水平对比,差异有统计学意义(P<0.05)。进行两两比较中度、重度的miR-223相对表达量均低于轻度,差异有统计学意义(P<0.05),重度miR-223相对表达量低于中度,差异有统计学意义(P<0.05)。中度、重度的MMP-9表达水平均高于轻度,差异有统计学意义(P<0.05),重度的MMP-9表达水平高于中度,差异有统计学意义(P<0.05)。外周血miR-223相对表量及MMP-9表达水平在预测OSAHS病情程度的AUC面积为:miR-223(0.696)、MMP-9(0.832);以最大约登指数计算出miR-223及MMP-9最大AUC面积的截断值,其中miR-223截断值为0.637(敏感度=73.30%,特异性=84.70%),MMP-9截断值为2297.323 ng/L(敏感度=86.20%,特异性=91.30%)。结论OSAHS患者外周血miR-223呈低表达状态,MMP-9呈高表达状态,与患者病情严重程度密切相关。Objective To investigate the expression and significance of microRNA-223 and MMP-9 in peripheral blood of patients with obstructive sleep apnea-hypopnea syndrome.Methods 50 patients with OSAHS admitted to our hospital from January 2017 to December 2018 were selected as the observation group,and 50 healthy people with health examination during the same period were selected as the control group.Sleep apnea hypopnea index(AHI)was used to classify the severity of the disease group,including 17 mild cases(AHI<10 times/h),22 moderate cases(10 times/h>AHI<20 times/h)and 11 severe cases(AHI>20 times/h).The levels of microRNA-223 in peripheral blood were detected by fluorescence quantitative PCR and MMP-9 in peripheral blood by enzyme-linked immunosorbent assay.The expression levels of microRNA-223 and MMP-9 in peripheral blood of the two groups were observed,and the differences between the two groups were compared.The expression levels of mir-223 and MMP-9 in OSAHS with different severity were analyzed.The ROC curve was used to analyze the efficacy of mir-223 and MMP-9 in predicting the severity of OSAHS.Results The relative expression of microRNA-223 in peripheral blood of patients in the observation group was significantly lower than that of the control group,while the expression of MMP-9 was significantly higher than that of the control group(P<0.05).The relative expression of miR-223 and the expression of MMP-9 in patients with OSAHS were mild(P<0.05).The relative expressions of moderate and severe miR-223 were lower than mild(P<0.05),and the relative expression of severe miR-223 was lower than moderate(P<0.05).The expression of moderate and severe MMP-9 was higher than mild(P<0.05),and the expression of severe MMP-9 was higher than moderate(P<0.05).The AUC area predicting the severity of OSAHS by the relative expression of microRNA-223 and MMP-9 in peripheral blood was:microRNA-223(0.696)and MMP-9(0.832).The truncation value of the maximum AUC area of microRNA-223 and MMP-9 was calculated by the most approximate lan
