腹腔镜广泛子宫切除术在治疗FIGO ⅠB～ⅡA期大瘤体宫颈癌中的临床价值

摘　　要：目的探讨腹腔镜下广泛子宫切除术在治疗大瘤体宫颈癌中的临床价值。方法回顾性分析40例国际妇产科联盟(FIGO)ⅠB~ⅡA期大瘤体(肿瘤直径>3 cm)宫颈癌患者,根据手术方式不同分为腹腔镜广泛子宫切除组(LRH组)和经腹广泛子宫切除组(ARH组),每组20例。比较2组患者术中情况、围手术期并发症及术后恢复情况等。结果LRH组术中失血量、输血率、肛门排气时间、术后住院时间均显著低于ARH组(P<0.05)。LRH组腹腔引流时间>ARH组(P<0.05)。LRH组总并发症发生率为55.0%显著低于ARH组的85.0%(P<0.05)。2组患者随访期无病生存情况差异无统计学意义(P>0.05)。结论腹腔镜下广泛全子宫切除术是治疗FIGOⅠB~ⅡA期大瘤体宫颈癌的一种可行方法。Objective To explore the clinical value of laparoscopic radical hysterectomy in treating large tumor cervical carcinoma during the stage of FIGOⅠB~ⅡA.Methods Forty patients with cervical cancer in the stage of FIGOⅠB~ⅡA(tumor diameter>3 cm)were divided into laparoscopic radical hysterectomy group(LRH group,n=20)and transabdominal radical hysterectomy group(ARH group,n=20).The conditions during and after operation as well as the perioperative complications of the patients were observed and compared between the two groups.Results The intraoperative blood loss,transfusion rate,anal exhaust time and hospital stay in LRH group were significantly lower than those in ARH group(P<0.05).Moreover,the time of peritoneal drainage in LRH group was longer than that in ARH group(P<0.05).The incidence of perioperative complications in LRH group(55.0%)was significantly lower than that(85.0%)in ARH group(P<0.05).In addition,there was no significant difference in disease-free survival condition during follow-up between two groups(P>0.05).Conclusion The laparoscopic radical hysterectomy is a feasible method in treating large tumor cervical carcinoma during the stage of FIGOⅠB~ⅡA.
