膝关节置换术引流管位置对疼痛的影响

摘　　要：目的分析全膝关节置换术中引流管放置位置的不同对术后引流管处疼痛的影响。方法收集2018年5~7月行单侧全膝关节置换术的患者资料,记录患者的性别、年龄、不同时段引流管处伤口的视觉疼痛模拟评分(VAS)和引流量、AKS评分等。将引流管置于膝关节外侧的患者30例为试验组,引流管置于膝关节内侧的患者27例为对照组,比较分析2组患者的VAS评分,肿胀值和引流液情况及AKS评分。结果2组患者术后12 h和72 h VAS评分、肿胀值、引流量和AKS评分比较差异无统计学意义(P>0.05);试验组术后24 h和48 h的VAS评分低于对照组,差异有统计学意义(P<0.05)。结论全膝关节置换术中引流管放置在膝关节外侧比内侧更能减轻患者引流管伤口处的疼痛,有助于患者的快速康复,值得临床应用。Objective To compare and analyze the effects of different positions of drainage tube on postoperative pain of patients during total knee replacement(TKR).Methods The clinical data of patients who underwent unilateral TKR in our department from May 2018 to July 2018 were analyzed.The data including patient’s gender and age,visual analogue pain score(VAS),drainage volume of the wound at drainage tube in different time points and AKS score were recorded.The patients with drainage tube placed on the lateral knee joint were enrolled as the experimental group,and those with drainage tube placed on the medial knee joint were enrolled control group.The general data were analyzed and compared between the two groups.Results A total of 57 patients were enrolled in the study,including 30 cases in experimental group and 27 cases in the control group.There were no significant differences in patients’average age and sex composition ratio between the two groups(P>0.05).There were no significant differences in VAS scores,swelling value,drainage volume and AKS scores at 12h and 72h after operation betweeen the two groups(P>0.05).However the VAS scores in experimental group at 24h,48h after operation were significantly lower than those in control group(P<0.05).Conclusion Placing drainage tube on the lateral knee joint during TKA can more effectively alleviate the pain at the wound of drainage tube more effectively,as compared with placing drainage tube on the medial knee joint,which is helpful for the rapid recovery process of patients.
