经脐单孔腹腔镜阑尾切除术治疗小儿阑尾炎的临床效果

摘　　要：目的探讨经脐单孔腹腔镜阑尾切除术治疗小儿阑尾炎的临床效果。方法选取2017年1月至2019年1月长治市妇幼保健院收治的阑尾炎患儿86例,随机分为两组,每组43例,观察组采用经脐单孔腹腔镜阑尾切除术进行治疗,对照组采用传统多孔腹腔镜阑尾切除术治疗。观察并比较两组患儿的术中、术后情况及术后并发症发生情况。结果观察组手术时间、术后排气时间、下床活动时间均短于对照组[(73.67±4.23)min比(82.48±6.39)min,(22.17±1.80)h比(28.05±2.46)h,(12.19±3.08)h比(17.13±4.40)h],差异有统计学意义(P<0.05);观察组术后并发症发生率(4.65%)低于对照组(23.26%),差异有统计学意义(P<0.05)。结论经脐单孔腹腔镜阑尾切除术治疗小儿阑尾炎是一种可行、效果确切的方法。Objective To investigate the clinical effectiveness of laparoscopic appendectomy on pediatric appendicitis via umbilicus.Methods A total of 86 children with appendicitis admitted to Changzhi Maternal and Child Health Hospital from January 2017 to January 2019 were selected.And all the patients were randomly divided into two groups,with 43 cases in each group.The observation group was treated with single-site laparoscopic surgery,while the control group was treated with traditional porous laparoscopic surgery.The intraoperative and postoperative conditions and complications of the two groups were observed and compared.Results The operation time,exhaust time and out-of-bed activity time of the observation group were(73.67±4.23)min,(22.17±1.80)h and(12.19±3.08)h,respectively,shorter than the(82.48±6.39)min,(28.05±2.46)h,(17.13±4.40)h of the control group,and the difference was statistically significant(P<0.05).The incidence of postoperative complications in the observation group(4.65%)was significantly lower than that in the control group(23.26%),P<0.05.Conclusions Transumbilical laparoscopic appendectomy for appendicitis in children is a feasible and effective method.
