联合检测血清降钙素原及C-反应蛋白在成人尿路感染患者中的应用价值

摘　　要：目的探讨成人尿路感染患者外周血清降钙素原(PCT)及C-反应蛋白(CRP)变化水平的应用价值。方法随机选取于本院确诊并接受治疗的尿路感染患者141例(感染组),选取同期本院健康体检者30例(正常对照组),检测PCT和CRP水平,对其结果用Wilcoxon秩和检验进行统计学分析并绘制ROC曲线。结果感染组PCT、CRP结果均明显高于对照组,差异有统计学意义(P <0. 05)。在检出率比较中,以PCT 0. 5 ng/ml、CRP 8 mg/L为限,分成≥0. 5 ng/ml和<0. 5 ng/ml 2组及≥8 mg/L和<8 mg/L 2组,结果显示PCT检出率为71. 6%,CRP检出率为65. 2%,两者共同检出率为90. 7%。在敏感性比较中,PCT以升高5倍以上的例数最多,而CRP以升高1倍以上最多,两者均以升高10倍以上最少。PCT曲线下面积为0. 903 (95%CI:0. 898~0. 969),CRP曲线下面积为0. 752 (95%CI:0. 638~0. 786)。结论联合检测血清PCT和CRP在成人尿路感染患者中具有较好的诊断价值,对病情治疗、观察及预后提供实验室依据,降低患者感染及并发症。Objective To explore the application value of the level changes of serum procalcitonin( PCT) and C-reactive protein( CRP) in adult patients with urinary tract infection. Methods A total of 141 patients with urinary tract infection diagnosed and treated in our hospital were randomly selected as the experimental group( infection group),and 30 examinees were selected during the same period( control group),the levels of PCT and CRP were detected and analyzed by Wilcoxon rank sum test and the ROC curve. Results The results of PCT and CRP in the infection group were significantly higher than those in the control group,and the differences were statistically significant( P < 0. 05). In terms of detection rate,PCT( 0. 5 ng/ml)and CRP( 8 mg/L) were taken as the boundary and divided into two groups: ≥0. 5 ng/ml or < 0. 5 ng/ml,and ≥8 mg/L or < 8 mg/L;the results showed that the detection rate of PCT was 71. 6%,CRP was 65. 2%,and that of the both was 90. 7%. In the sensitivity comparison,cases with a PCT increase of more than 5 times were the most,while cases with a CRP increase of more than 1 times were the most,and cases with a 10 times or more increase were the least. The area under the PCT curve was 0. 903( 95% CI: 0. 898 to 0. 969),and the area under the CRP curve was 0. 752( 95% CI: 0. 638 to 0. 786).Conclusion Combined detection of PCT and CRP have a good diagnostic value in adult patients with urinary tract infection,and provides laboratory basis for disease diagnose,treatment,observation and prognosis to reduce infection and complications.
