白细胞介素-6、血清淀粉样A蛋白、唾液酸、HBP在发热患者中的诊断价值

摘　　要：目的探讨白细胞介素-6(IL-6)、肝素结合蛋白(HBP)、血清淀粉样蛋白A(SAA)、以及唾液酸(SA)在发热患者中的诊疗意义。方法选取2018年1月-5月发热患者125例(观察组),同时选取体检健康体检者40例(对照组)测定其血清SAA、CRP、SA和IL-6的水平,运用ROC曲线评价其在各组中的诊断意义。结果 IL-6、SAA、HBP及SA在细菌感染组的水平均高于非细菌感染组和对照组,差异有统计学意义(F=16. 82,F=33. 83,F=26. 13,F=29. 35,P均=0. 00)。HBP的曲线下面积(AUC)最大,为0. 925,IL-6和SAA分别为0. 864、0. 842,SA最低为0. 728,4项指标联合AUC为0. 839。HBP的AUC大于其余3项指标(P <0. 05)。HBP、IL-6、SAA、SA其诊断细菌感染的截断值分别为11. 1 ng/ml、7 pg/ml、38 mg/L、68. 7 mg/dl,其诊断细菌感染的敏感度为88. 1%、79. 76%、79. 8%、69%;特异度为87. 2%、82. 06%、74. 4%、66. 7%。结论联合检测血清HBP、IL-6、SAA、SA对发热患者的临床诊断有重要的意义。Objective To explore the clinical significance of interleukin-6( IL-6),heparin binding protein( HBP),serum albuminoid protein A( SAA),and sialic acid( SA) in patients with fever. Methods A total of 125 patients with fever from January to May 2018( observation group) were selected. At the same time,40 healthy people( control group) who underwent physical examination were selected to determine their serum levels of SAA,CRP,SA,and IL-6. ROC curve was used to evaluate the diagnostic significance in the groups. Results The levels of IL-6,SAA,HBP and SA in the bacterial infection group were all higher than those in the non-bacterial infection group and the control group,showing statistical significance( F = 16. 82,F = 33. 83,F = 26. 13,F = 29. 35,P = 0. 00). The area( AUC) under the curve of HBP is the largest,which is0. 925,IL-6 and SAA are 0. 864 and 0. 842 respectively,SA is the lowest,which is 0. 728,and AUC combined with four indicators is 0. 839. The AUC of HBP was greater than the other three indexes( P < 0. 05). The truncation values of HBP,IL-6,SAA and SA in the diagnosis of bacterial infection were 11. 1 ng/ml,7 pg/ml,38 mg/l and 68. 7 mg/dl,respectively.The specificity was 87. 2%,82. 06%,74. 4% and 66. 7%. Conclusion The combined detection of serum HBP,IL-6,SAA and SA is of great significance to the clinical diagnosis of febrile patients.
