摘  要:目的:比较利奈唑胺与万古霉素治疗新生儿革兰阳性球菌肺炎的疗效及安全性。方法:选取2016年10月至2019年3月广西壮族自治区民族医院新生儿科收治的48例确诊或疑似革兰阳性球菌肺炎患儿进行回顾性分析,根据用药不同分为利奈唑胺组和万古霉素组,每组24例。比较两组患儿临床疗效、病原菌培养情况、实验室指标水平及不良反应发生情况的差异。结果:利奈唑胺组、万古霉素组患儿的总有效率分别为87. 5%(21/24)、83. 3%(20/24),差异无统计学意义(P>0. 05)。利奈唑胺组、万古霉素组患儿的病原菌培养阳性率分别为37. 5%(9/24)、54. 2%(13/24),治疗结束后,两组患儿革兰阳性球菌均转为阴性。治疗前后,两组患儿中性粒细胞绝对值、红细胞计数、血小板计数、总胆红素、丙氨酸氨基转移酶及血清肌酐水平的差异均无统计学意义(P>0. 05)。利奈唑胺组1例患儿出现血小板计数降低,万古霉素组患儿未见不良反应发生。结论:利奈唑胺与万古霉素治疗新生儿革兰阳性球菌肺炎的疗效相当,利奈唑胺个别病例出现血小板计数降低,用药期间应定期监测。OBJECTIVE: To compare the efficacy and safety of linezolid and vancomycin in the treatment of neonatal Gram-positive coccal pneumonia. METHODS: Totally 48 children with confirmed or suspected Gram-positive coccal pneumonia admitted into Guangxi Zhuang Autonomous Region National Hospital from Oct. 2016 to Mar. 2019 were extracted to be divided into linezolid group and vancomycin group via different treatment regimen,with 24 cases in each group. The clinical efficacy,the status of pathogen culture,the level of experimental indicators and adverse drug reactions of two groups were compared. RESULTS: The total effective rates of the linezolid group and vancomycin group were 87. 5%( 21/24) and 83. 3%( 20/24),respectively,with no significant difference( P > 0. 05). The positive rates of pathogenic culture in the linezolid group and vancomycin group were 37. 5%( 9/24) and 54. 2%( 13/24),respectively. After treatment,Gram-positive cocci were negative in both groups. Before and after treatment,there were no statistically significant differences in the absolute values of neutrophils,erythrocyte count,platelet count,total bilirubin,alanine aminotransferase and serum creatinine levels of two groups( P>0. 05). One child in the linezolid group had decreased platelet count,while no adverse drug reactions were observed in the vancomycin group.CONCLUSIONS: The efficacy of linezolid and vancomycin in the treatment of neonatal gram-positive coccal pneumonia is similar. Decreased platelet count was observed in individual cases in the linezolid group,which should be monitored regularly during medication.
