PDCA循环管理在外科活血化瘀类中药注射剂临床应用中的成效探讨

摘　　要：目的:探讨PDCA循环管理在南京市浦口医院(以下简称"我院")外科活血化瘀类中药注射剂临床应用中的成效,促进临床合理用药。方法:运用PDCA模式发现活血化瘀中药注射剂在我院不合理使用现状,分析原因,制订改进措施,落实监管与检查和持续性改进;分析并比较PDCA循环管理干预前后活血化瘀中药注射剂的使用率、销售金额、用药频度(defined daily dose system,DDDs)等数据及不合理使用情况。结果:经过2个阶段的PDCA循环管理干预,我院活血化瘀类中药注射剂的销售金额、销售金额占药品总销售金额的比例及DDDs均较干预前明显降低,差异均有统计学意义(P<0.05);第2阶段PDCA循环管理干预后,我院外科活血化瘀类中药注射剂的使用率明显低于第1阶段及干预前,差异均有统计学意义(P<0.05)。结论:运用PDCA循环管理模式,能够促进我院外科活血化瘀类中药注射剂合理应用及管理水平提高。OBJECTIVE:To probe into the effect of PDCA cycle management on the application of traditional Chinese medicine injection for activating blood and dissolving stasis in surgery department in the Pukou Hospital of Nanjing(hereinafter referred to as"our hospital"),and to promote clinical rational medication.METHODS:PDCA model was adopted to find out the irrational application of traditional Chinese medicine injection for activating blood and dissolving stasis in our hospital,so as to analyze the reason,formulate improvement measures,implement supervision and inspection and continuous improvement;the usage rate,consumption sum,defined daily dose system(DDDs)and irrational application of traditional Chinese medicine injection for activating blood and dissolving stasis were analyzed and compared before and after the implementation of PDCA.RESULTS:After 2 stages of PDCA intervention,the accumulative consumption sum and its proportion and accumulative DDDs of traditional Chinese medicine injection for activating blood and dissolving stasis in our hospital had been significantly lower than those of before intervention,with statistically significant differences(P<0.05);after the second stage of PDCA intervention,the usage rate of traditional Chinese medicine injection for activating blood and dissolving stasis in surgery department in our hospital had been significantly lower than that of the first stage and before intervention,with statistically significant differences(P<0.05).CONCLUSIONS:The application of PDCA cycle management can promote rational application and management of traditional Chinese medicine injection for activating blood and dissolving stasis in surgery department in our hospital.
