摘  要:目的探讨不同体质量指数(BMI)的冠心病患者心包周围脂肪含量的差异及意义。方法选择冠心病患者127例,按照患者BMI分为正常体质量组17例、超重组74例、肥胖组36例。应用德国西门子第二代Somatom Definition Flash炫速双源CT半自动后处理Volume软件测量各组心外膜脂肪(EAT)及心外膜旁脂肪(PAT),观察冠状动脉形态不自然的发生情况。结果肥胖组、超重组的EAT、PAT均高于正常体质量组,且肥胖组的EAT、PAT均高于超重组(P均<0.05或0.01)。BMI与EAT、PAT均呈正相关(r分别为0.464、0.404,P均<0.001)。肥胖组中男性EAT高于女性(P<0.05)、PAT低于女性(P<0.01)。超重组及肥胖组冠状动脉形态不自然的发生率高于正常体质量组(χ2=9.001,P=0.011)。结论随着BMI的增加,冠心病患者EAT与PAT均明显增大,且与冠状动脉形态学改变有一定关系。Objective To investigate the difference and significance of pericardial fat volume in coronary heart disease patients with different body mass index(BMI).Methods A total of 127 patients with coronary heart disease were selected and divided into the normal body mass group(17 cases),overweight group(74 cases),and obesity group(36 cases)according to BMI.Epicardial adipose tissue(EAT)and pericardial adipose tissue(PAT)in each group were measured by the semi-automatic Volume software of Somatom Definition Flash,the second generation of Siemens,Germany,and then we observed the occurrence of abnormal coronary artery morphology.Results The obesity group and the overweight group had higher EAT and PAT than the normal body mass group,and the obesity group had higher EAT and PAT than the overweight group(all P<0.05 or P<0.01).BMI was positively correlated with EAT and PAT(r=0.464 and 0.404,respectively,both P<0.001).In the obesity group,the EAT in males was higher than that in females(P<0.05)and PAT was lower than that in females(P<0.01).The incidence of abnormal coronary artery morphology in the overweight group and obesity group was higher than that in the normal body mass group(χ2=9.001,P=0.011).Conclusion With the increase of BMI,both EAT and PAT increase significantly in patients with coronary heart disease,which is related to the morphological changes of coronary arteries.
