摘　　要：目的比较Firehawk(火鹰)支架和EXCEL支架在冠心病治疗中的安全性和有效性及治疗前后骨膜蛋白(Periostin,POSTN)水平的变化。方法选取2014年6月至2017年6月河北省胸科医院收治的冠心病患者96例,均成功实施经皮冠状动脉介入治疗(PCI)治疗。根据药物洗脱支架类型将所有患者分为两组,观察组48例,均使用火鹰国产靶向洗脱支架植入治疗,对照组48例,均使用Excel药物洗脱支架植入治疗。均随访2年,比较两组患者POSTN水平、结局变量(致死性心肌梗死、全因死亡率、靶病变血运重建率、NYHA心功能分级等情况)。结果对照组术前POSTN为(3.18±1.28)ng/ml,术后为(2.42±0.94)ng/ml;观察组术前POSTN为(3.16±1.29)ng/ml,术后为(2.39±0.95)ng/ml,两组患者术后POSTN水平较术前均有明显改善(P<0.05),但术前、术后两组POSTN水平比较,差异未见统计学意义(P>0.05);两组心肌梗死、全因死亡、靶血管血运重建等发生率和NYHA心功能分级比较,差异未见统计学意义(P>0.05)。结论 Firehawk支架与Excel支架植入均能改善冠心病患者POSTN水平,两者结局变量无明显差异,均可在冠心病PCI术中应用。Objective To compare the safety and efficacy of Firehawk and EXCEL stents in the treatment of coronary heart disease and the changes of Periostin(POSTN) levels before and after treatment. Methods Ninety-six patients with coronary heart disease admitted to Hebei Chest Hospital from June 2014 to June 2017 were selected. All patients were successfully treated with percutaneous coronary intervention(PCI). According to the types of drug-eluting stents, all patients were divided into two groups, 48 patients in the observation group were treated with targeting eluting stents made by Firehawk, while 48 patients in the control group were treated with Excel drug-eluting stents. All patients were followed up for 2 years. POSTN levels and outcome variables(fatal myocardial infarction, all-cause mortality, target lesion revascularization rate, NYHA classification of cardiac function) were compared between the two groups. Results The POSTN level was(3.18±1.28) ng/ml before operation and(2.42±0.94) ng/ml after operation in the control group. The POSTN level was(3.16±1.29) ng/ml before operation and(2.39±0.95) ng/ml after operation in the observation group. The POSTN levels in the two groups were significantly improved after operation(P<0.05), but there was no significant difference in POSTN levels between the two groups before and after operation(P>0.05). There was no significant difference in the incidence of death, target vessel revascularization or NYHA cardiac function classification between the two groups(P>0.05). Conclusions Firehawk drug eluting stent and Excel drug eluting stent can decrease the periostin of coronary artery disease patients. The outcome variable of the two groups without significant difference. Both of them can be used in the interventional treatment of coronary artery disease.
