摘 要:【目的】检测急性肺栓塞患者外周血中低氧诱导因子-1α(hypoxia inducible factor-1 alpha,HIF-1α)与凝血因子X(coagulation factor X,FX)、Ⅶ(coagulation factor Ⅶ,FⅦ)活性,并探讨HIF-1α与FX、FⅦ之间的关系及意义。【方法】选取2015年3月至2018年2月因肺栓塞住院接受治疗的160例患者为研究对象,其中86例为急性肺栓塞患者(APE组),74例为慢性肺栓塞患者(CPE组);同期选取53例健康体检者(CG组)作为对照组。采用酶联免疫吸附法(enzyme-linked immunosorbent assay,ELISA)检测3组外周血中的HIF-1α、FX及FⅦ水平,同时检测D-二聚体(D-dimer,D-Di)、肺动脉收缩压(pulmonary artery systolic pressure,PASP)、动脉血氧分压(arterial partial pressure of oxygen,PaO2)以及平均动脉压(mean arterial pressure,MAP)等肺栓塞相关指标;分析HIF-1α、FX、FⅦ以及肺栓塞相关指标在急性肺栓塞患者外周血中的相关性,分析影响急性肺栓塞的因素。【结果】APE组外周血中的HIF-1α、FX、FⅦ、D-Di以及PASP水平显著高于CPE组和CG组(P<0.05),PaO2、MAP水平显著低于CPE组和CG组(P<0.05);肺栓塞患者外周血中HIF-1α、FX、FⅦ活性与D-Di、PASP均呈现正相关(P<0.05),与PaO2、MAP均呈现负相关(P<0.05)。多因素结果显示HIF-1α、FX以及D-Di是影响急性肺栓塞的危险因素,PaO2是影响急性肺栓塞的保护因素。【结论】急性肺栓塞患者外周血中HIF-1α与FX、FⅦ均呈现正相关,三者与急性肺栓塞病情发展密切相关,HIF-1α、FX、FⅦ结合肺栓塞相关指标对评价急性肺栓塞的早期危险性有重要意义。【Objective】To explore the relationship between hypoxia inducible factor-1 alpha(HIF-1α) and coagulation factor X(FX),coagulation factor Ⅶ(FⅦ) and its significance by measuring serum levels of HIF-1α and FX, FⅦ in patients with acute pulmonary embolism.【Methods】A total of 160 patients with pulmonary embolism hospitalized from March 2015 to February 2018 were selected as the study subjects, and 86 of them were cases with acute pulmonary embolism(APE group), 74 of them were patients with chronic pulmonary embolism(CPE group). At the same time, 53 healthy check-ups(CG group) were selected as the control group.The levels of HIF-1α, FX and FVⅡ in peripheral blood of patients in the three groups were detected by enzyme-linked immunosorbent assay(ELISA). Pulmonary embolism related indexes such as D-dimer(D-Di), pulmonary artery systolic pressure(PASP), arterial partial pressure of oxygen(PaO2) and mean arterial pressure(MAP) were also measured. The correlations of HIF-1α, FX, FVⅡ and related indexes of pulmonary embolism in peripheral blood of patients with acute pulmonary embolism were analyzed, and the factors affecting acute pulmonary embolism were also analyzed.【Results】The levels of HIF-1α, FX, FVⅡ, D-Di and PASP in peripheral blood in the APE group were significantly higher than those in the CPE group and CG group(P < 0.05), while the levels of PaO2 and MAP were significantly lower than those in the CPE group and CG group(P <0.05). The activities of HIF-1α, FX and FVⅡ in peripheral blood of patients with pulmonary embolism were correlated with D-Di, PASP, PaO2 and MAP(P < 0.05). Multivariate results showed that HIF-1α, FX and D-Di were risk factors for acute pulmonary embolism.【Conclusion】The levels of HIF-1α, FX and FVⅡ in the peripheral blood of patients with acute pulmonary embolism increased significantly, which is closely related to the development of acute pulmonary embolism.
