2型糖尿病患者红细胞流变学与糖化血红蛋白水平的相关性

摘  要:目的探讨2型糖尿病患者红细胞流变学与糖化血红蛋白(HbA1c)水平的相关性。方法回顾性分析开封市中医院2018年5月-2019年3月收治的104例2型糖尿病患者临床资料,根据糖化血红蛋白不同将其分为两组,HbA1c<7%、血糖控制良好为A组(n=35),HbA1c≥7%、血糖控制不佳为B组(n=69)。分别抽取患者静脉血,检测并对比两组血液流变学指标及血糖水平,分析红细胞流变学与2型糖尿病患者HbA1c水平的相关性。结果 B组全血高切黏度、全血低切黏度、红细胞变形指数及FPG、2h PBG均高于A组,差异有统计学意义(P<0.05);全血高切黏度、全血低切黏度、FPG、2h PBG与2型糖尿病患者HbA1c水平呈正相关(r=0.657、0.384、0.568、0.498,P均<0.05),与红细胞变形指数呈负相关(r=-5.057,P<0.05)。结论全血高切黏度、全血低切黏度、FPG、2h PBG水平与2型糖尿病患者HbA1c水平呈正相关,即血糖水平控制越不理想,红细胞变形指数越低、糖尿病水平控制越好,因此红细胞流变学可为血糖控制方案的制定提供有效的参考依据。Objective To investigate the correlation between erythrocyte hemorheology and glycosylated hemoglobin(HbA1c)in patients with type 2 diabetes mellitus(T2 DM). Methods The clinical data of 104 patients with type 2 diabetes mellitus treatedin the hospital from May, 2018 to March, 2019 were analyzed retrospectively. According to the difference of glycosylated hemoglo-bin, they were divided into two groups: group A(n=35, HbA1c≥7%) and group B(n=69). Venous blood was taken from patients.Hemorheological indexes and blood glucose levels were measured and compared between the two groups. Erythrocyte hemorheologyand type 2 diabetes mellitus were analyzed. The correlation between erythrocyte hemorheology and glycosylated hemoglobin(HbA1c) in patients with type 2 diabetes mellitus(T2 DM) was analyzed. Results The whole blood high-cut viscosity, the wholeblood low-cut viscosity, the red blood cell deformation index and the FPG, 2 h PBG were higher than that in group A, and thedifference was statistically significant(P<0.05). The whole blood high-cut viscosity, the whole blood low-cut viscosity, the FPG,the 2 h PBG were positively correlated with the HbA1c level of the patients with type 2 diabetes(r=0.657, 0.384, 0.568 and 0.498,P<0.05). There was a negative correlation with the erythrocyte deformation index(r=-5.057, P<0.05). Conclusion There was a pos-itive correlation between whole blood high shear viscosity, whole blood low shear viscosity, FPG, the level of 2 h PBG and the levelof HbA1c in patients with type 2 diabetes mellitus, that is, the less satisfactory the control of blood glucose level is, the lower theerythrocyte deformability index and the better the control of diabetes level is. Therefore, erythrocyte hemorheology can provide aneffective reference for the formulation of blood glucose control program.
