重度子痫前期患者血清学指标、胎盘血流与肝素酶变化的相关性分析

摘　　要：目的探讨重度子痫前期患者血清学指标、胎盘血流与肝素酶变化的相关性。方法选择广州市花都区人民医院住院分娩的重度子痫前期的孕妇45例作为病例组,同期选择孕期健康和血压正常的孕妇125例作为对照组。检测两组血清学MMP与PLGF含量,采用PCR方法检测肝素酶相对表达量情况,超声检测血管血流指数(VFI)、血管指数(VI)、血流指数(flow index,FI)并进行相关性分析。结果病例组胎儿的双顶径、头围、腹围都显著低于对照组(P<0.05)。病例组孕妇的全血肝素酶相对表达量比对照组高,但PLGF、MMP含量比对照组低,两组对比有明显差异(P<0.05)。病例组孕妇的胎盘血流动力学指标VI、FI与VFI都显著高于对照组(P<0.05)。在病例组中,直线相关分析显示孕妇的全血肝素酶相对表达量与胎盘血流动力学指标都有显著正相关性(P<0.05),与血清MMP、PLGF含量有显著负相关性(P<0.05)。结论重度子痫前期患者血清MMP、PLGF表达量显著下降,肝素酶相对表达量上升,伴随有胎盘血流异常,三者存在相关性。Objective To investigate the correlation among serum parameters, placental blood flow and heparanase changes in patients with severe preeclampsia. Methods A total of 45 cases of pregnant women with severe pre-eclampsia who were hospitalized for delivery were selected as the case group. And the other 125 healthy pregnant women with normal blood pressure were selected as the control group. The serum MMP and PLGF levels were detected in both groups. The relative expression of heparinase were detected by PCR. Ultrasound detection and correlation analysis was done on the vascularization index(VI), vascularization flow index(VFI), blood flow index(FI) were given. Results The biparietal diameter, head circumference and abdominal circumference of the fetus in the case group were significantly lower than those in the control group(P<0.05). The relative expression of heparinase in the pregnant women in the case group were higher than that in the control group, while the serum MMP and PLGF levels were lower than the control group, and the difference were statistically significant(P<0.05). The placental hemodynamic parameters VI, FI and VFI of the pregnant women in the case group were significantly higher than those in the control group(P<0.05). In the case group, linear correlation analysis showed that the relative expression of whole blood heparinase in pregnant women were significantly positively correlated with placental hemodynamics(P<0.05), and were significant negative correlation with serum MMP and PLGF levels(P<0.05). Conclusion The expression of serum MMP and PLGF in patients with severe preeclampsia significantly decreases, the relative expression of heparinase increases, and the placental blood flow are abnorma.And there are correlation among the three indicators.
