摘 要:目的探讨经耳缘静脉注射脂肪干细胞修复兔激素性股骨头坏死的可行性及其修复效果。方法应用马血清联合激素法建立兔激素性股骨头坏死动物模型,随机选取36只造模成功的兔进行随机分组,分为静脉注射组、局部注射组及对照组,每组各12只兔。从3只非造模兔分离脂肪干细胞,传代至第3代,予溴脱氧尿嘧啶核苷标记后制成细胞悬液。静脉注射组经耳缘静脉注入5 ml脂肪干细胞悬液,局部注射组经关节局部注入同量脂肪干细胞,对照组不作任何干预。于干预后第4周、第8周各组随机选取6只模型兔,处死后切取左侧股骨头作切片,作组织学分析,并计算空骨陷窝率及计数Brdu阳性细胞数量。结果干预后4周,对照组可见较多空骨陷窝及脂肪细胞、骨小梁稀疏,而局部注射组、静脉注射组空骨陷窝及脂肪细胞数量较对照组少。干预后8周,对照组可见空骨陷窝及脂肪细胞数量较干预后4周增多,部分脂肪细胞已融合成团,可见骨小梁断裂;局部注射组及静脉注射组可见空骨陷窝及脂肪细胞数量极少,骨小梁较致密。干预后4周、8周局部注射组及静脉注射组的空骨陷窝率均高于对照组(P均<0.05),而局部注射组与静脉注射组比较差异无统计学意义(P>0.05)。干预后8周局部注射组及静脉注射组空骨陷窝率均低于同组干预后4周,差异有统计学意义(P<0.05)。干预后4周、8周,局部注射组及静脉注射组可见Brdu标记的阳性细胞,而对照组均未见经Brdu标记的阳性细胞。干预后8周局部注射组及静脉注射组Brdu阳性细胞数均高于同组干预后4周(P均<0.05),而两组同一时间点作比较,差异无统计学意义(P>0.05)。结论经静脉注射ADSCs可定植于股骨头区,并对兔SANFH有一定的修复作用。Objective To investigate the feasibility and effect of adipose-derived stem cells(ADSCs)intravenously injected through ear margin for the repair of steroid-induced avascular necrosis of femoral head in rabbits.Methods The animal model of steroid-induced femoral head necrosis in rabbits should be established by horse serum combined with hormone method.36 successful rabbits were randomly divided into intravenous injection group,local injection group and control group,with 12 rabbits in each group.Adipose stem cells were isolated from three non-model rabbits and subcultured to the third generation.Cell suspension was made by labeling with bromodeoxyuridine.In the intravenous injection group,5 ml adipose stem cell suspension was injected through the ear margin vein,and in the local injection group,the same amount of adipose stem cells were injected into the joint,while in the control group,no intervention was given.At the 4 th and 8 th weeks after intervention,6 rabbits were randomly selected from each group.After execution,the left femoral head was cut for histological analysis,and the empty bone lacunae rate was calculated and the number of Brdu positive cells was counted.Results At 4 weeks after intervention,there were more empty bone lacunae and adipocytes and sparse bone trabeculae in the control group,while the number of empty bone lacunae and adipocytes in the local injection group and the intravenous injection group was less than that in the control group.8 weeks after intervention:in the control group,the number of empty bone lacunae and adipocytes increased,some adipocytes had fused into clusters,and bone trabecular fracture could be seen;in the local injection group and intravenous injection group,the number of empty bone lacunae and adipocytes was very small,and the bone trabecula was denser.The rate of empty bone lacunae in local injection group and intravenous injection group was higher than that in control group 4 and 8 weeks after intervention(P<0.05),but there was no significant difference between lo
