摘 要:目的观察神经肌电图联合MRI诊断小儿臂丛神经损伤的临床价值。方法将2017年2月-2019年3月郑州大学附属儿童医院接治的疑似臂丛神经损伤患儿100例作为研究对象,入选患儿均接受神经肌电图联合MRI检查,观察联合检查结果,并以手术探查结果作为"金标准",分析神经肌电图联合MRI诊断的特异度、灵敏度及准确度。结果本次研究入选的100例疑似臂丛神经损伤患儿,经手术探查结果确认为臂丛神经损伤92例,其中上臂丛损伤39例,下臂丛损伤35例,全臂丛损伤18例;术前神经肌电图联合MRI检出臂丛神经损伤患儿86例,与手术结果符合84例;以手术探查结果作为金标准,MRI诊断灵敏度、特异度、准确度分别为80.43%、37.50%、77.00%,神经肌电图联合MRI诊断灵敏度、特异度、准确度分别为91.30%、75.00%、90.00%。结论神经肌电图联合MRI能够快速、准确的诊断小儿臂丛神经损伤,灵敏度及准确度较高,可为临床治疗提供确切的参考依据信息。Objective To observe the clinical value of electromyography combined with MRI in the diagnosis of brachial plexus injury in children.Methods 100 children with suspected brachial plexus nerve injury admitted to the hospital from February,2017 to March,2019 were selected as the study subjects.All the selected children underwent neuromyography combined with magnetic resonance imaging.The results of the combined examination were observed.The specificity,sensitivity and accuracy of diagnostic value of neuromyography combined with magnetic resonance imaging were analyzed with the results of surgical exploration as the"gold standard".Results Of the 100 suspected brachial plexus injuries,92 cases were confirmed by surgical exploration,including 39 cases of upper brachial plexus injuries,35 cases of lower brachial plexus injuries and 18 cases of total brachial plexus injuries.The diagnostic sensitivity,specificity and accuracy of MRI were 80.43%,37.50%and 77.00%,respectively.The diagnostic sensitivity,specificity and accuracy of EMG combined with MRI were 91.30%,75.00%and 90.00%,respectively.Conclusion Neuromyography combined with MRI can quickly and accurately diagnose brachial plexus nerve injury in children with high sensitivity and accuracy,which can provide accurate reference information for clinical treatment.
