食管超声对心房颤动患者左心房/左心耳血栓的诊断价值

摘　　要：目的探讨食管超声对心房颤动患者左心房/左心耳血栓的诊断价值。方法选取2016年11月-2018年11月于郑州市第七人民医院接受治疗的心房颤动患者86例,均行食管超声检查,并以临床诊断结果为"金标准",评价食管超声检查诊断左心房/左心耳血栓的准确性、敏感性、特异性以及阳性预测值、阴性预测值。结果食管超声诊断左心房血栓的准确度为96.51%,敏感度为96.15%,特异度为97.06%,阳性预测值为98.04%,阴性预测值为94.29%;食管超声诊断左心耳血栓的准确度为96.51%,敏感度为96.00%,特异度为97.22%,阳性预测值为97.96%,阴性预测值为94.59%。结论食管超声在心房颤动患者左心房/左心耳血栓诊断中的应用价值较高,可作为判断是否有血栓形成的主要检查手段。Objective To investigate the diagnostic value of esophageal echocardiography in left atrial/left atrial appendage thrombosis in patients with atrial fibrillation.Methods 86 patients with atrial fibrillation treated in the hospital from November,2016 to November,2018 were examined by esophageal ultrasonography,and the clinical diagnosis results were regarded as"gold standard".The accuracy,sensitivity,specificity,positive predictive value and negative predictive value of esophageal ultrasonography in the diagnosis of left atrial/left atrial appendage thrombosis were evaluated.Results The accuracy,sensitivity,specificity,positive predictive value and negative predictive value of esophageal echocardiography in the diagnosis of left atrial thrombosis were96.51%,96.15%,97.06%,98.04%,94.29%,96.51%,96.00%,97.22%,97.96%and 94.59%,respectively.Conclusion Esophageal echocardiography is of high value in the diagnosis of left atrial/left atrial appendage thrombosis in patients with atrial fibrillation,and can be used as the main examination method to judge whether there is thrombosis or not.
